摘要
目的比较瑞马唑仑与丙泊酚对胃癌合并2型糖尿病(T2DM)患者血糖及胰岛素水平的影响。方法选取2020年3月至2022年10月张掖市第二人民医院收治的120例胃癌合并T2DM患者作为研究对象,随机分为瑞马唑仑组和丙泊酚组,每组60例。比较两组术中情况,不同时间点血糖、胰岛素水平及变化幅度。结果两组麻醉时间、手术时间、术中出血量、胶体用量、晶体用量比较差异无统计学意义(P>0.05)。两组在麻醉前、麻醉30 min、麻醉60 min、麻醉120 min、麻醉150min、拔管时的血糖水平比较差异无统计学意义(P>0.05),但在麻醉90min的血糖水平比较,瑞马唑仑组明显低于丙泊酚组,差异有统计学意义(P<0.05)。两组在麻醉60min、麻醉120min及拔管时的血糖变化值比较差异无统计学意义(P>0.05),但在麻醉30 min、麻醉90 min、麻醉150 min的血糖变化值比较,差异有统计学意义(P<0.05)。两组不同时点胰岛素水平比较差异无统计学意义(P>0.05)。两组在麻醉30 min、麻醉90 min、麻醉120 min、麻醉150 min及拔管时的胰岛素变化值比较差异无统计学意义(P>0.05),但在麻醉60min的胰岛素变化值比较差异有统计学意义(P<0.05)。结论瑞马唑仑与丙泊酚对胃癌合并T2DM患者术中血糖和胰岛素水平的影响基本无差异,均能将血糖控制在安全范围内。
Objective To compare the effects of Remazolam and Propofol on blood glucose and insulin levels in gastric cancer patients with type 2 diabetes(T2DM).Methods A total of 120 patients with gastric cancer complicated with T2DM admitted to Zhangye Second People's Hospital from March 2020 to October 2022 were selected as the study objects and randomly divided into remazolam group and propofol group,with 60 cases in each group.The intraoperative conditions,blood glucose and insulin levels at different time points and the range of changes were compared between the two groups.Results There was no significant difference in anesthesia time,operation time,intraoperative blood loss,colloidal dosage and crystal dosage between the two groups(P>0.05).There was no significant difference in blood glucose levels between the two groups before anesthesia,30 min of anesthesia,60 min of anesthesia,120 min of anesthesia,150 min of anesthesia,and at extubation(P>0.05),but at 90 min of anesthesia,the blood glucose level of remazolam group was significantly lower than that of propofol group.The difference was statistically significant(P<0.05).There was no significant difference in blood glucose changes between the two groups at 60 min,120 min and extubation(P>0.05),but there was significant difference in blood glucose changes at 30 min,90 min and 150 min(P<0.05).There was no significant difference in insulin levels between the two groups at different time points(P>0.05).There was no significant difference in insulin changes between the two groups at 30 min,90 min,120 min,150 min and extubation(P>0.05),but there was significant difference in insulin changes at 60 min(P<0.05).Conclusion Remazolam and propofol have no difference in the effects on blood glucose and insulin levels during operation in gastric cancer patients with T2DM,and both can control blood glucose within a safe range.
