摘要
1例64岁老年男性患者,因“乏力1周,发热伴呼吸困难半日”入院,患者罹患天疱疮3个月,目前口服泼尼松30 mg,qd维持治疗,入院诊断为呼吸衰竭、重症肺炎、天疱疮等。入院后给予哌拉西林钠他唑巴坦钠联合左氧氟沙星、卡泊芬净抗感染,后因血液二代基因测序结果提示耶氏肺孢子菌,改用复方磺胺甲[口恶]唑。临床药师参与抗感染方案及糖皮质激素给药的调整,配合医生进行个体化治疗,并给予全程药学监护。患者经治后病情平稳,准予出院。
A 64-year-old male patient was admitted due to fatigue for 1 week and fever with dyspnea for half a day and diagnosed with respiratory failure,severe pneumonia,and pemphigus.He had a 3-month history of pemphigus and was currently treated with prednisone(30 mg,qd).After admission,piperacillin sodium and tazobactam sodium,levofloxacin and caspofungin were given to him,which was changed to trimethoprim-sulfamethoxazole according to the result of next-generation sequencing(NGS)of blood showing pneumocystis jirovecii.Clinical pharmacists made individualized regimens by adjusting anti-infection drugs and dose of glucocorticoid,and carry out the whole process of pharmaceutical care.The patient was discharged in stable condition.
