基于序列的多重聚合酶链反应对献血者HLA/HPA基因分型

摘要 目的研究嘉兴地区献血者HLA-I类(A、B位点)和HPA基因表达情况,建立本地区血小板基因数据库。方法采用基于序列的多重聚合酶链反应(multiplex PCR-SBT)对嘉兴地区5,411例样本进行HLA-I类(A、B)基因测序,对其中199例单采血小板献血者样本进行HPA1-6、15、21w系统基因分型,统计分析HLA-I类和HPA1-6、15、21w基因频率分布情况。结果5,411例样本中共检出HLA-A、B位点等位基因53种,其中HLA-A位点等位基因15个,频率前5位为A*2(0.3040)、A*11(0.2384)、A*24(0.1771)、A*33(0.0893)、A*30(0.0493);HLA-B位点等位基因38个,频率前5位为B*46(0.1137)、B*60(0.1136)、B*13(0.0931)、B*62(0.0822)和B*58(0.0820)。HPA1~6、15、21w系统均符合Hardy-Weinberg遗传平衡定律,具有恒定性,以HPA-3杂合度(a/b杂合子基因型)最高(0.482),其次为HPA-15(0.437),HPA-3、15系统中a、b抗原频率相近,HPA-3、15系统有b/b纯合子基因型,分别为0.196,0.266。其余5个系统均以a/a纯合子基因型为主,分别为0.995、0.8995、0.990、0.975、0.945,未见b/b基因型。结论HLA-A、B位点和HPA3、15系统基因具有多态性分布特点,建立献血者HLA/HPA基因数据库可有效解决血小板输注无效问题。 Objective To study the expression of human leukocyte antigen-1(HLA-I)(site A and B)and HPA genes in blood donors in Jiaxing and to establish a preliminary platelet gene database.Methods Sequence-based multiplex PCR-SBT was used to sequence HLA-I(site A and B)genes in 5,411 samples from blood donors in Jiaxing,and 199 samples from single-collection platelet donors were genotyped for human platelet antigen(HPA)1-6,15,21w systems,and statistical analysis was performed.The frequency distribution of HLA-I and HPA1-6,15,21w genes were statistically analyzed.Results A total of 53 HLA-A and B locus alleles were detected in 5,411 blood donor samples,among which 15 HLA-A locus alleles with the top 5 frequencies of A*2(0.3040),A*11(0.2384),A*24(0.1771),A*33(0.0893)and A*30(0.0493).There were 38 alleles in HLA-B locus,and the first five alleles were B*46(0.1137),B*60(0.1136),B*13(0.0931),B*62(0.0822)and B*58(0.0820).The HPA1-6,15,21w systems all conformed to the Hardy-Weinberg law,with the HPA-3 heterozygosity(a/b heterozygous genotype)was the highest(0.482),followed by HPA-15(0.437).In HPA-3 and 15 systems,a and b antigen frequencies were similar,and HPA-3 and 15 systems had b/b pure heterozygous genotypes,0.196,0.266 respectively.a/a pure heterozygous genotypes dominated the remaining five systems,0.995,0.8995,0.990,0.975,0.945,and no b/b genotypes were seen.Conclusion The HLA-A、B loci and HPA3,15 system genes of blood donors are characterized by polymorphic distribution,and the establishment of platelet HLA/HPA gene database can effectively solve the problem of ineffective platelet transfusion.
