摘要
目的对比观察侧卧位与截石位对于足月初产妇第二产程护理效果的影响。方法选取60名足月初产妇,根据随机数字表法分为研究组与对照组,各30名。对照组于第二产程采取截石位,研究组于第二产程采取侧卧位,比较两组第二产程时间、产后不同时间点疼痛评分及产后不良结局。结果研究组第二产程时间短于对照组,产后2h、4h、8h的视觉模拟评分法(VAS)评分均低于对照组,差异有统计学意义(P<0.05)。研究组新生儿窒息、会阴损伤以及会阴侧切发生率(10.00%、13.33%、3.33%)均低于对照组(36.67%、43.33%、26.67%),差异有统计学意义(P<0.05)。结论对于足月初产妇,于第二产程护理时采取侧卧位可使产程时间缩短,不仅能够缓解其会阴疼痛,还能减少新生儿窒息、会阴损伤、会阴侧切等不良结局,临床应用价值高于截石位,值得推广。
Objective To observe the effect of lateral decubitus position and lithotomy position on the second stage of labor.Methods 60 full-term primipara were selected and divided into study group and control group with 30 cases in each group according to random number table method.The control group took lithotomy position during the second stage of labor,and the study group took lateral position during the second stage of labor.The pain scores and postpartum adverse outcomes of the two groups were compared at different time points of the second stage of labor.Results The second stage of labor in the study group was shorter than that in the control group,and the VAS scores at 2 h,4 h and 8 h postpartum were lower than those in the control group,with statistical significance(P<0.05).The incidence of neonatal asphyxia,perineal injury and lateral perineal incision(10.00%,13.33%,3.33%)in the study group was lower than that in the control group(36.67%,43.33%,26.67%),and the difference was statistically significant(P<0.05).Conclusion For full-term parturient parturient women,lateral decubitus position during the second stage of labor can shorten the labor time,not only relieve perineal pain,but also can reduce neonatal asphyxia,perineal injury,lateral perineal incision and other adverse outcomes.The clinical application value is higher than lithotomy position,worthy of promotion.
