超声弹性成像联合超声TI-RADS分级诊断和鉴别甲状腺结节良恶性的价值

Value of Ultrasound Elastography Combined with Ultrasonic TI-RADS in the Diagnosis and Differentiation of Benign and Malignant Thyroid Nodules

摘要 目的:分析超声弹性成像与超声甲状腺影像报告和数据系统(TI-RADS)分级诊断在甲状腺结节良恶性鉴别和诊断中的应用价值。方法:选取2020年6月—2022年5月湖南航天医院收治的甲状腺结节患者71例(93个结节)作为研究对象,患者均实施超声弹性成像检查和TI-RADS分级检查,以手术病理检查结果为诊断“金标准”,分析超声弹性成像、TL-RADS分级及超声弹性成像联合TI-RADS分级对甲状腺结节诊断结果,比较单项检测结果与联合检测的诊断效能。结果:超声弹性成像及超声弹性成像联合TI-RADS分级诊断在甲状腺结节良恶性诊断中的灵敏度与准确度均高于TI-RADS分级诊断,差异有统计学意义(P<0.05);超声弹性成像与超声弹性成像联合TI-RADS分级诊断在甲状腺结节良恶性诊断中的特异度比较,差异无统计学意义(P>0.05);超声弹性成像联合TI-RADS分级诊断的特异度高于TI-RADS分级,差异有统计学意义(P<0.05);超声弹性成像与超声弹性成像联合TI-RADS分级诊断在甲状腺结节良恶性诊断中的灵敏度比较,差异无统计学意义(P>0.05)。结论:超声弹性成像在甲状腺结节良恶性诊断中的价值高于超声TI-RADS分级诊断,联合诊断可进一步降低漏诊与误诊风险。 Objective:To analyze the application value of ultrasonic elastography and ultrasonic thyroid image report and data system(TI-RADS)in differential diagnosis of benign and malignant thyroid nodules.Methods:A total of 71 patients with thyroid nodules(93 nodules)were selected as the study objects in the Hunan Aerospace Hospital from June 2020 to May 2022.All patients underwent ultrasound elastography and TI-RADS grading examination,and the results of surgical pathological examination were taken as the"gold standard"for diagnosis.Ultrasonic elastography,TL-RADS grading and ultrasonic elastography combined with TI-RADS grading were analyzed for the diagnosis of thyroid nodules,and the diagnostic efficiency of single detection and combined detection was compared.Results:The sensitivity and accuracy of ultrasound elastography and ultrasound elastography combined with TI-RADS grading diagnosis in the diagnosis of benign and malignant thyroid nodules were higher than those of TIRADS grading diagnosis,and the difference was statistically significant(P<0.05);there was no significant difference in the specificity of ultrasound elastography and ultrasound elastography combined with TI-RADS in the diagnosis of benign and malignant thyroid nodules(P>0.05);the specificity of ultrasonic elastic imaging combined with TI-RADS classification was higher than that of TI-RADS classification,and the difference was statistically significant(P<0.05);there was no significant difference in sensitivity between ultrasound elastography and ultrasound elastography combined with TI-RADS in the diagnosis of benign and malignant thyroid nodules(P>0.05).Conclusion:The value of ultrasound elastography in the diagnosis of benign and malignant thyroid nodules is higher than that of ultrasound TI-RADS classification diagnosis,and combined diagnosis can further reduce the risk of missed diagnosis and misdiagnosis.
