摘要
目的探究磁共振成像(MRI)在小儿下丘脑错构瘤(HH)诊断中的应用价值。方法选取郑州大学附属儿童医院2020年1月至2021年12月收治的疑似HH患儿10例,分别进行CT和MRI检查,以病理检查为金标准分析其诊断价值。结果以病理检查为金标准,MRI诊断的敏感度、特异度分别为100%和75.00%,CT诊断的敏感度、特异度分别为83.33%和75.00%,MRI诊断的敏感度明显高于CT(P<0.05);6例HH患儿中,包括Ⅰ型患儿1例,临床表现主要为性早熟;Ⅱ型患儿1例,Ⅲ型患儿4例,临床表现主要为痴笑样癫痫。接受手术或药物治疗后,性早熟患儿激素水平恢复正常,痴笑样癫痫患儿癫痫发作次数明显减少。结论MRI能够对HH患儿进行准确的诊断和临床分型,从而为治疗方式和手术方式的选择提供重要支持。
【Objective】To explore the application value of MRI in the diagnosis of hypothalamic hamartoma(HH)in children.【Methods】A total of 10 children with suspected HH admitted to Children's Hospital of Zhengzhou University from January 2020 to December 2021 were selected for CT and MRI examination,and their diagnostic value was analyzed by pathological examination as the gold standard.【Results】Taking pathological examination as the gold standard,the sensitivity and specificity of MRI diagnosis were 100%and 75.00%,respectively,and the sensitivity and specificity of CT diagnosis were 83.33%and 75.00%,respectively,and the sensitivity of MRI diagnosis was significantly higher than that of CT(P<0.05).Among the 6 children with HH,1 child with type I was included,and the main clinical manifestation was precocious puberty.There was 1 case of type II and 4 cases of type III,and the clinical manifestation was mainly smirk-like epilepsy.After surgery or drug treatment,the hormone levels of children with precocious puberty returned to normal,and the number of seizures in children with simpering epilepsy was significantly reduced.【Conclusion】MRI can accurately diagnose and classify children with HH,thus providing important support for the choice of treatment and surgical method.
