摘要
目的:观察益肝退黄汤联合西药治疗酒精性肝病并发胆汁淤积的临床疗效。方法:选择酒精性肝病并发胆汁淤积患者402例,随机分为对照组和观察组各201例。两组均进行常规治疗,对照组在常规治疗基础上给予熊去氧胆酸、脂溶性维生素,观察组在对照组基础上给予益肝退黄汤。比较两组治疗前后的血清生化指标水平和肝纤维化指标水平,评价临床疗效。结果:治疗4周后,两组患者血清丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、碱性磷酸酶(ALP)、γ-谷氨酰转肽酶(GGT)、总胆红素(TBil)、直接胆红素(DBil)、甘胆酸(CG)、胆汁酸(BA)水平较治疗前均降低(P<0.05),且观察组患者上述生化指标水平均低于对照组(P<0.001);两组患者血清层粘连蛋白(LN)、透明质酸(HA)、Ⅳ型胶原蛋白(Ⅳ-Col)、Ⅲ型前胶原(PCⅢ)水平较治疗前均降低(P<0.05),且观察组患者上述肝纤维化指标水平均低于对照组(P<0.001)。观察组、对照组总有效率分别为87.56%、78.61%,两组临床疗效比较,差异有统计学意义(P<0.05)。结论:益肝退黄汤联合西药治疗酒精性肝病并发胆汁淤积疗效显著,能有效改善患者肝功能和肝纤维化状况。
Objective:To observe the clinical curative efficacy of Yigan Tuihuang Decoction(益肝退黄汤)combined with western medicine in the treatment of alcoholic liver disease complicated with cholestasis.Methods:A total of 402 patients with alcoholic liver disease complicated with cholestasis were selected and randomly divided into control group and observation group,with 201 cases in each group.Both groups received conventional treatment.The control group was given ursodeoxycholic acid and liposoluble vitamins on the basis of conventional treatment,while the observation group was given Yigan Tuihuang Decoction on the basis of the control group.The levels of serum biochemical indicators and liver hepatic indicators were compared between the two groups before and after treatment,and the clinical curative efficacy of the two groups were evaluated.Results:After 4 weeks of treatment,the levels of serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),gamma glutamyl transpeptidase(GGT),total bilirubin(TBil),direct bilirubin(DBil),cholyglycine(CG),and bile acid(BA)in the two groups were decreased compared to those before treatment(P<0.05),and the levels of these biochemical indicators in the observation group were lower than those in the control group(P<0.001).The levels of serum laminin(LN),hyaluronic acid(HA),typeⅣcollagen(Ⅳ-Col),and typeⅢprocollagen(PCⅢ)in the two groups were decreased compared to those before treatment(P<0.05),and the levels of these hepatic fibrosis indicators in the observation group were lower than those in the control group(P<0.001).The total effective rate of the observation group and the control group was 87.56%and 78.61%,respectively.The difference in clinical curative efficacy between the two groups was statistically significant(P<0.05).Conclusions:The combination of Yigan Tuihuang Decoction and western medicine has significant curative efficacy on alcoholic liver disease complicated with cholestasis,which can effectively improve the patients’liver function and hepatic fibrosis.
