泪液趋化因子CCL19及CCL21在干眼诊断中的临床应用研究

摘要 目的 探索趋化因子配体19(CCL19)和趋化因子配体21(CCL21)在干眼患者泪液中的表达情况和临床相关性,评估其在干眼中是否具有诊断价值。方法 干眼的诊断基于《中国干眼专家共识:检查和诊断(2020年)》。将2022年4—11月在贵州医科大学附属医院眼科干眼门诊就诊的36例女性干眼患者(36眼)设为观察组,将同期36例健康女性(36眼)设为对照组。所有受检者均在每日上午同一时段,由同一医师采用眼表疾病指数评分法(OSDI)进行干眼临床症状评估,采用角膜荧光素染色(FL)评分、泪膜破裂时间(BUT)和泪液分泌试验(SⅠt)进行干眼临床体征检测。同时,采用液相芯片检测技术(Luminex)检测受检者泪液中CCL19及CCL21的表达水平,通过统计学方法分析其与干眼临床症状及体征的相关性。结果 观察组泪液中CCL19及CCL21表达水平显著高于对照组,差异有统计学意义(P <0.05)。泪液中CCL19及CCL21表达与OSDI、FL呈正相关,与BUT、SⅠt呈负相关。CCL19诊断干眼的灵敏度为0.97,特异度为0.92;CCL21诊断干眼的灵敏度为0.94,特异度为0.92。结论 泪液CCL19和CCL21在干眼中具有潜在的诊断价值。 Objective To explore the expression and clinical relevance of C-C motif chemokine ligand 19(CCL19)and CCL21 in the tears of dry eye patients,and to assess their diagnostic performance in dry eye.Methods The diagnosis of dry eye was based on the Expert Consensus on Dry Eye in China:Examination and Diagnosis(2020).Thirty-six female dry eye patients(36 eyes)visiting the dry eye clinic of the Affiliated Hospital of Guizhou Medical University between April 2022 and November 2022 were set as the observation group,and 36 healthy females(36 eyes)during the same period were set as the control group.All subjects were evaluated by a single physician at the same time every morning.The severity of dry eye symptoms was evaluated using the Ocular Surface Disease Index(OSDI)assessment,while dry eye signs were evaluated using tear film break-up time(BUT),SchirmerⅠtest(SⅠt),and corneal fluorescein staining(FL).The expression levels of CCL19 and CCL21 in the tears of the subjects were measured using Luminex liquid chip detection system,and their correlation with clinical symptoms and signs of dry eye was analyzed using statistical methods.Results The expression levels of CCL19 and CCL21 in tears of the observation group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).The expression of CCL19 and CCL21 in tears was positively correlated with OSDI and FL,and negatively correlated with BUT and SⅠt.The sensitivity value of CCL19 and CCL21 was 0.97 and 0.94,respectively,while the specificity value of CCL19 and CCL21 was both 0.92 for the diagnosis of dry eye.Conclusion Tear CCL19 and CCL21 have potential diagnostic value in dry eye.
