高强度有氧运动联合呼吸训练对COPD稳定期患者运动耐力的影响 被引量:1

Effect of the combination of high-intensity aerobic exercise with a combined breathing training on exercise tolerance of patients with chronic obstructive pulmonary disease in stable stage

摘要 目的探讨高强度有氧运动联合组合式呼吸训练对慢性阻塞性肺疾病(COPD)稳定期患者运动耐力的影响。方法本研究采用前瞻性随机对照分析,研究对象纳入2021年1月至2022年3月首都医科大学附属北京康复医院收治的COPD稳定期患者150例,随机数字表分为对照组和干预组,每组75例。对照组患者给予常规护理干预,干预组患者在此基础上给予高强度有氧运动联合组合式呼吸训练。比较2组患者干预前后运动耐力、肺功能[呼气高峰流量(PEFR)、用力肺活量(FVC)、1 s率:第1秒用力呼气容积(FEV_(1))与FVC的比值]及生活质量情况[世界卫生组织生存质量量表(WHOQOL-BREF)、COPD综合评估测试(CAT)]。结果2组患者干预前运动耐力、FVC、PEFR、1 s率FEV_(1)/FVC、WHOQOL-BREF、CAT评分比较,差异无统计学意义(P>0.05);干预后,干预组患者6 min步行距离明显高于对照组(P<0.05);干预组肺功能FVC、PEFR、FEV_(1)/FVC水平明显高于对照组(P<0.05);干预组患者WHOQOL-BREF评分明显高于对照组,CAT评分明显低于对照组(P<0.05)。结论强度有氧运动联合组合式呼吸训练可以提高COPD稳定期患者的运动耐力,改善肺功能,提高生活质量。 Objective To explore the effect of high-intensity aerobic exercise with a combined breathing training on exercise tolerance of patients with chronic obstructive pulmonary disease(COPD)in stable stage.Methods A prospective randomized controlled analysis was performed involving 150 COPD patients in stable stage who were admitted to Beijing Rehabilitation Hospital Affiliated to Capital Medical University from January 2021 to March 2022.They were randomly divided into control group and intervention group,with 75 patients in each group.Routine nursing intervention was given to all patients,and those in the intervention group were additionally given the combination intervention of high-intensity aerobic exercise with a combined breathing training.The exercise endurance,lung function(peak expiratory flow rate[PEFR],forced vital capacity[FVC],forced expiratory volume in 1s[FEV1]/FVC)and quality of life(WHO Quality of Life-BREF[WHOQOL-BREF],COPD Assessment Test[CAT])before and after the intervention were compared between groups.Results There were no significant differences in the exercise endurance,FVC,PEFR,FEV1/FVC,WHOQOL-BREF score and CAT score before intervention between groups(P>0.05).After intervention,the 6-minute walking distance of patients in the intervention group was significantly longer than that in the control group(P<0.05).The FVC,PEFR,and FEV1/FVC after intervention were significantly higher in the intervention group than those of control group(P<0.05).After intervention,the WHOQOL-BREF score was significantly higher,and the CAT score was significantly lower in the intervention group than those of control group(P<0.05).Conclusion The combination of high-intensity aerobic exercise with a combined breathing training significantly improves exercise tolerance,lung function and quality of life in COPD patients in stable stage.
