摘要
目的探讨不同年龄儿童BMI指数及贫血与龋齿的相互作用。方法前瞻性选取2019年8月至2022年8月进行口腔检查的150例学龄前儿童作为调查对象,收集其临床资料,测量所有研究对象的体重、身高,并对其血红蛋白水平进行检测,采用speraman相关性分析法分析龋齿数与儿童各生育指标的相关性。根据儿童是否患有龋齿分为龋齿组(n=63)和无龋齿组(n=87),通过单因素和多因素分析方法分析儿童BMI及贫血与龋齿的相互作用。结果共150例学龄前儿童总龋齿率为42.00%,其中4~5岁儿童的龋齿率为56.45%、3~4岁为35.00%、2~3岁为25.00%,组间对比,4~5岁儿童的龋齿率较其他年龄组明显偏高(P<0.05)。各年龄段患儿的BMI、血红蛋白水平比较,差异均无统计学意义(P>0.05)。不同BMI组儿童的dmft指数、dmft+DMFT指数比较差异有统计学意义(P<0.05),其中肥胖组儿童的dmft指数、dmft+DMFT指数最低。Spearman相关性分析结果显示,BMI与dmft指数、dmft+DMFT指数呈负相关(R=-0.063、-0.520,P<0.05)。根据儿童的血红蛋白水平差异对其进行分组,其中正常76例,贫血64例。血红蛋白水平正常组儿童的dmft指数、dmft+DMFT指数均明显低于贫血组儿童(P<0.05)。Spearman相关性分析结果显示,血红蛋白与dmft指数、dmft+DMFT指数呈正相关(R=0.454、0.865,P<0.001)。Logstic回归分析结果显示,儿童偏瘦、贫血及家庭年收入≤3万元是影响其罹患龋齿的独立危险因素(P<0.05)。结论BMI偏低和发生贫血会明显增加学龄前儿童发生龋齿的风险,而龋齿的发生则会进一步阻碍幼儿的正常发育,提示应注意定期进行口腔检查。
Objective To explore the interaction of body mass index(BMI)and anemia with dental caries in children of different ages.Methods A total of 150 pre-school children who underwent oral examination in our hospital from August 2019 to August 2022 were prospectively selected as the investigation objects,and their clinical data were collected.The body weight,height and hemoglobin level of all children were measured.The correlation between the number of dental caries and development indicators of children was identified by Spearman correlation analysis.Children were divided into caries group(n=63)and caries-free group(n=87)according to whether they had dental caries or not.The univariate and multivariate analysis were performed to analyze the interaction of the body mass index(BMI)and anemia with dental caries.Results The total rate of dental caries in 150 preschool children was 42.00%,which,in children with 4-5 years of age,3-4 years of age and 2-3 years of age,was 56.45%,35.00%,and 25.00%,respectively.Compared with that of other age groups,the dental caries rate of children with 4-5 years of age was significantly higher(P<0.05).There were no significant differences in the BMI and hemoglobin level among children of all ages(P>0.05).There were significant differences in the decay-missing-filled in deciduous teeth(dmft)index and dmft+decay-missing-filled in permanent teeth(dmft+DMFT)index in children subgrouped by BMI(P<0.05),and the lowest dmft index and dmft+DMFT index were detected in obese children.Spearman correlation analysis showed that BMI was negatively correlated with dmft index and dmft+DMFT index(R=-0.063,and-0.520,respectively;P<0.05).Subgrouped by the hemoglobin level,children were divided into normal group(n=76)and anemia group(n=64).The dmft index and dmft+DMFT index of children in the normal group were significantly lower than those in the anemia group.Spearman correlation analysis showed that hemoglobin level was positively correlated with dmft index and dmft+DMFT index(R=0.454,and 0.865,respectively;P<0.001).Logistic regression analysis showed that thinness,anemia and family income≤30,000 yuan were independent risk factors for dental caries(P<0.05).Conclusion Low BMI and anemia significantly increase the risk of dental caries in preschool children,and the occurrence of dental caries further hinders their normal development.It is important to take regular oral examinations in preschool children.
