摘要
目的探讨绝经后2型糖尿病患者发生骨质疏松的影响因素。方法选取2021年1月—2022年1月于莆田学院附属医院体检科就诊的324名绝经后2型糖尿病(type 2 diabetes mellitus,T2DM)患者为研究对象,根据骨密度检测结果,分为骨量正常组169例,骨量减少组109例,骨质疏松组46例,比较3组患者年龄、血压、血脂、空腹血糖、空腹胰岛素、糖化血红蛋白、糖尿病病程、绝经年限、胰岛素抵抗等情况。采用多因素Logistic回归分析绝经后2型糖尿病患者骨质疏松的影响因素。结果3组患者的年龄、空腹胰岛素、糖化血红蛋白、糖尿病病程、绝经年限、稳态模型胰岛素抵抗指数比较,差异有统计学意义(P<0.05)。Logistic分析结果显示,年龄、空腹胰岛素、糖化血红蛋白、糖尿病病程、绝经年限、稳态模型胰岛素抵抗指数是骨质疏松的独立危险因素(P<0.05)。结论高龄,空腹胰岛素增高,血糖控制不佳,糖尿病病程、绝经年限久以及胰岛素抵抗的绝经后2型糖尿病患者更应注意预防骨质疏松的发生。
Objective To explore the influencing factors of osteoporosis in postmenopausal patients with type 2 diabetes.Methods 324 postmenopausal type 2 diabetes mellitus(T2DM)patients who visited the Department of Endocrinology,Affiliated Hospital of Putian University from January 2021 to January 2022 were selected as the research subjects.According to the bone density test results,169 patients were divided into a normal bone mass group,109 patients in an osteopenia group,and 46 patients in an osteoporosis group.The age,blood pressure,blood lipids,fasting blood glucose,fasting insulin,glycosylated hemoglobin,duration of diabetes,years of menopause,and insulin resistance were compared among the three groups of patients.Multivariate Logistic regression was used to analyze the influencing factors of osteoporosis in postmenopausal patients with type 2 diabetes.Results There were statistically significant differences in age,fasting insulin,glycated hemoglobin,duration of diabetes,years of menopause,and steady-state model insulin resistance index among the three groups of patients(P<0.05).Logistic analysis results showed that age,fasting insulin,glycated hemoglobin,duration of diabetes,years of menopause,and homeostatic model insulin resistance index were independent risk factors for osteoporosis(P<0.05).Conclusion Postmenopausal patients with type 2 diabetes who are advanced in age,have elevated fasting insulin,poor blood glucose control,diabetes duration,long menopausal years,and insulin resistance should pay more attention to preventing the occurrence of osteoporosis.
