摘要
目的系统评价中药口服汤剂联合正骨手法治疗颈椎病的有效性与安全性。方法检索中国知网、维普数据库、万方数据库、PubMed及Web of Science数据库2000年1月1日至2022年8月15日的相关文献,纳入所有中药口服汤剂联合正骨手法治疗颈椎病的临床随机对照试验,由2名研究者独立筛选文献、提取文献中有效数据,而后采用RevMan 5.4进行文章质量评估与meta分析。结果本研究共纳入14篇文献,涉及患者1667例。meta分析结果显示,与单纯采用中药口服汤剂或正骨手法治疗相比,中药口服汤剂与正骨手法联合使用能够提高疗效[RR=1.16,95%CI(1.12~1.21),P<0.01],改善患者颈椎疼痛[SMD=-1.18,95%CI(-2.98~-0.79),P<0.01],缓解患者颈性眩晕症状[SMD=1.55,95%CI(0.59~2.52),P<0.01],改善临床整体症状[SMD=-2.04,95%CI(-3.29~-0.79),P<0.01]。结论中药口服汤剂联合正骨手法治疗颈椎病能够提高临床效果,缓解患者颈部疼痛、僵硬状态,改善颈性眩晕等全身症状,从而提高患者生活质量。
Objective To systematically evaluate the efficacy and safety of oral decoction of traditional Chinese medicine combined with bone setting manipulation in the treatment of cervical spondylosis.Methods The relevant literatures from January 1,2000 to August 15,2022 in CNKI,VIP database,Wanfang database,PubMed and Web of Science database were retrieved,and all the clinical randomized controlled trials of oral decoction of traditional Chinese medicine combined with bone setting manipulation in treating cervical spondylosis were included.Two researchers independently screened the literatures,extracted the effective data in the literatures,and then used RevMan 5.4 for article quality evaluation and meta analysis.Results A total of 14 articles were included,involving 1667 patients.Meta analysis results showed that compared with the simple treatment of traditional Chinese medicine oral decoction or bone setting manipulation,the combination of traditional Chinese medicine oral decoction and bone setting manipulation could improve the curative effect[RR=1.16,95%CI(1.12-1.21),P<0.01],improve patients’cervical pain[SMD=-1.18,95%CI(-2.98--0.79),P<0.01],and relieve patients’cervical vertigo symptoms[SMD=1.55,95%CI(0.59~2.52),P<0.01],the overall clinical symptoms were improved[SMD=-2.04,95%CI(-3.29--0.79),P<0.01].Conclusion Traditional Chinese medicine oral decoction combined with bone setting manipulation can improve the clinical efficacy of cervical spondylosis,relieve the pain and stiffness of the neck,improve the general symptoms such as cervical vertigo,and thus improve the quality of life of patients.
