摘要
目的分析呼吸介入治疗后转入呼吸重症监护病房(RICU)患者的临床特征,以加强呼吸介入治疗医师对这类患者的认识及预后判断。方法回顾性总结2022年1月1日至2022年12月31日本院因呼吸介入治疗围手术期并发症入住RICU的患者信息,评估患者RICU住院时间、应用抗生素、气管插管、机械通气的特点及病情转归。结果共纳入58例患者,麻醉复苏延迟是呼吸介入治疗后入住RICU的首要原因(38/58,65.52%)。RICU住院中位时间是6.00(3.00,8.00)d,56例静脉应用抗生素(56/58,96.55%),抗感染中位疗程是5.00(3.00,8.00)d。其中气管插管状态入科40例(40/58,68.97%),有创机械通气时间69.00(45.00,119.00)h,相对无气管插管入科患者,其气道病变更广泛、抗生素应用天数及RICU住院天数更长(均P<0.05)。38例(38/40,95.00%)成功拔除气管套管并撤机,拔管成功率95.00%。结论因呼吸介入治疗围手术期并发症无法回普通病房患者,RICU可为其提供后续生命支持,为患者顺利度过围手术期危险保驾护航。
Objective Analyze the clinical characteristics of patients transferred to the respiratory intensive care unit(RICU)after respiratory intervention therapy,in order to enhance the understanding and prognosis judgment of respiratory intervention therapists for such patients.Methods A retrospective analysis was conducted on the clinical data of patients admitted to RICU of our hospital from January 1,2022 to December 31,2022.The characteristics of patient RICU hospitalization time,antibiotic application,tracheal intubation,mechanical ventilation and patients’outcome were evaluated.Results A total of 58 cases were included,delayed anesthesia resuscitation is the primary cause of admitting to RICU after respiratory intervention therapy(38/58,65.52%).The length of hospitalization in RICU was 6.00(3.00,8.00)days,with 56 cases receiving intravenous antibiotics(56/58,96.55%)and 5.00(3.00,8.00)days receiving antibiotic treatment.Among them,40 patients(40/58,68.97%)were admitted with tracheal intubation,and the invasive mechanical ventilation time was 69.00(45.00,119.00)hours.They had a wide range of airway diseases(P<0.05),longer days of antibiotic use(P<0.05)and hospitalization in RICU(P<0.05),compared with patients without tracheal intubation.38 cases(38/40,95.00%)successfully removed the tracheal cannula and evacuated the ventilator.The success rate of removing tracheal cannula was 95.00%.Conclusions For patients who are unable to return to the regular ward due to perioperative complications caused by respiratory intervention treatment,RICU can provide follow-up life support to help them successfully overcome the perioperative risks.
