摘要
目的探讨血清游离三碘甲腺原氨酸(FT_(3))、游离四碘甲状腺素(FT_(4))、促甲状腺激素(TSH)单项指标及三项联合检测对临床诊断甲状腺功能的效能。方法选取2019年3月至2021年3月在本院就医并确诊为甲状腺功能障碍的患者共80例,根据患者甲状腺功能障碍程度分为亚甲减组(n=42)及甲减组(n=38);另选取同期在本院接受体检的健康受试者30例纳入正常对照组。纳入本研究的所有对象均取其清晨空腹静脉血,离心结束后将获得的各上清样本置于Cobas e411全自动电化学发光免疫分析仪对血清中FT_(3)、FT_(4)、TSH水平进行检测。对比各组血清FT_(3)、FT_(4)、TSH指标水平;采用Spearman相关系数分析上述三项指标与甲状腺功能障碍程度的相关性;采用ROC分析上述三项指标与甲状腺功能障碍的相关性。结果各组血清FT_(3)、FT_(4)、TSH水平对比均具有统计学差异(P<0.05),随着甲状腺功能障碍的加剧,患者血清FT_(3)、FT_(4)水平呈低表达趋势发展,患者血清TSH水平呈高表达趋势发展(均P<0.05);采用Spearman相关系数分析显示血清FT_(3)、FT_(4)与甲状腺功能减退呈负相关(均P<0.05),血清TSH与甲状腺功能减退呈正相关(P<0.05);采用ROC对血清FT_(3)、FT_(4)、TSH进行分析,结果显示3项指标单项诊断效能对比TSH的AUC值更高,但3项指标联合检测的AUC值最高(均P<0.05)。结论血清FT_(3)、FT_(4)、TSH联合检测对评估和诊断甲状腺功能障碍等具有重要意义,对临床准确评估病情并制订相应的诊疗方案提供有效依据。
Objective To investigate the efficacy of serum free triiodothyronine(FT_(3)),free tetraodothyronine(FT_(4))and thyroid-stimulating hormone(TSH)in clinical diagnosis of thyroid function.Methods A total of 80 patients diagnosed with thyroid dysfunction in our hospital from March 2019 to March 2021 were selected,including 42 patients with subclinical hypothyroidism(included in the subclinical hypothyroidism group)and 38 patients with hypothyroidism(included in the subclinical hypothyroidism group).In addition,30 healthy subjects who received physical examination in our hospital during the same period were included in the normal control group.Fasting venous blood of all subjects included in this study was collected in the morning.After centrifugation,the obtained supernatant samples were placed into the Cobas e411 automatic electrochemiluminescence immunoanalyzer to detect serum FT_(3),FT_(4)and TSH levels.Serum FT_(3),FT_(4)and TSH levels were compared among all groups.Spearman correlation coefficient was used to analyze the correlation between serum FT_(3),FT_(4),TSH and thyroid dysfunction.ROC was used to analyze the correlation between serum FT_(3),FT_(4),TSH levels and thyroid dysfunction.Results There were statistical differences in serum FT_(3),FT_(4)and TSH levels among all groups(P<0.05).With the aggravation of hypothyroidism,serum FT_(3)and FT_(4)levels of patients showed a significant decreasing trend,while serum TSH level of patients showed a significant increasing trend(all P<0.05).Spearman correlation coefficient analysis showed that serum FT_(3)and FT_(4)were negatively correlated with hypothyroidism(all P<0.05),and serum TSH was positively correlated with hypothyroidism(P<0.05).ROC was used to analyze serum FT_(3),FT_(4)and TSH.The results showed that the AUC value of the three indexes was higher than that of TSH,but the AUC value of the combined detection of the three indexes was the highest(all P<0.05).Conclusions The combined detection of serum FT_(3),FT_(4)and TSH is of great significance for the assessment and diagnosis of thyroid dysfunction,and provides an effective basis for accurate clinical assessment and formulation of corresponding diagnosis and treatment plans.
