摘要
目的分析卵巢子宫内膜异位囊肿合并不孕患者腹腔镜手术后自然妊娠有关影响因素。方法选择商丘市第五人民医院在2018年1月至2021年1月期间收治卵巢子宫内膜异位囊肿合并不孕总共82例患者进行研究,均接受2年时间随访,结合术后有无自然妊娠分成妊娠组(n=50)和未妊娠组(n=32),比较两组各项临床资料差异,对存在差异的指标开展Logistic回归分析,分析影响术后自然妊娠的主要因素。结果两组体重指数、不孕类型、囊肿位置、囊肿直径、痛经程度、术后辅助用药相比,差异无统计学意义(P>0.05);但年龄、不孕时间、囊肿数量、r-AFS分期以及EFI相比,差异有统计学意义(P<0.05),其中未妊娠组的年龄≥35岁、不孕时间≥3年、囊肿多发、r-AFS分期Ⅳ期以及EFI<7分占比均高于妊娠组,差异有统计学意义(P<0.05)。对存在差异的指标开展Logistic回归分析,发现年龄、不孕时间、囊肿数量、r-AFS分期以及生育指数均是影响腹腔镜手术后自然妊娠的主要因素,差异有统计学意义(P<0.05)。结论卵巢子宫内膜异位囊肿伴不孕患者腹腔镜手术后自然妊娠的影响因素较多,主要包含年龄、不孕时间、囊肿数量、疾病分期以及生育指数等,这些指标在评估术后妊娠结局中有着重要作用。
Objective To analyze the influence factors of postoperative spontaneous pregnancy in ovarian endometriotic cyst women with infertility following the laparoscopic surgery.Methods 82 ovarian endometriotic cyst women with infertility treated from January 2018 to January 2021 in our hospital were investigated.During two-year follow-up visits,women were assigned to pregnancy group(n=50,with spontaneous pregnancy)and non-pregnancy group(n=32,without spontaneous pregnancy).Logistic regression equation was used to analyze the major influence factors of postoperative spontaneous pregnancy.Results There were no significant between-group differences about the body mass index,infertility types,cyst sites,cyst diameter,dysmenorrhea degrees and adjuvant drug usage(P>0.05);the patient’s age,infertility time,cyst number,disease staging and fertility index were found to be statistically significant between groups(P<0.05);The proportion of age≥35 years,infertility duration≥3 years,multiple cysts,disease stage Ⅳ,and fertility index<7 points in the non pregnant group were higher than those in the control group(P<0.05).Logistic regression analysis was conducted on the indicators with differences,and it was found that age,infertility time,number of cysts,r-AFS staging,and fertility index were the main factors affecting natural pregnancy after laparoscopic surgery,with OR values>1 and P<0.05.Conclusion After the laparoscopic surgery,there are multiple influence factors of spontaneous pregnancy,such as the patient’s age,infertility time,cyst number,disease staging and fertility index,etc.The above mentioned indicators are important to predict the pregnancy outcomes for ovarian endometriotic cyst women with infertility.
