低频体外膈肌起搏器联合呼吸功能锻炼对慢性阻塞性肺病急性加重期患者肺功能和膈肌功能的影响 被引量:4

摘要 目的探讨低频体外膈肌起搏器联合呼吸功能锻炼对慢性阻塞性肺病急性加重期(AECOPD)患者肺功能和膈肌功能的影响。方法选取2022年1月至8月医院治疗的AECOPD患者84例,采用随机数字表法随机分为两组,每组42例。两组均予常规药物治疗、人工叩背排痰及持续低流量吸氧等基础治疗。对照组予常规呼吸功能锻炼,观察组在对照组常规呼吸功能锻炼基础上加低频体外膈肌起搏器治疗,两组均连续治疗4周,比较两组患者治疗前后肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)及FEV1占预计值的百分比(FEV1%)]、临床症状指标[改良版呼吸困难程度量表(mMRC)评分和COPD调查问卷(CAT)评分]和膈肌功能指标[呼气末膈肌厚度、吸气末膈肌厚度和膈肌活动度]的变化情况。结果治疗后,两组FEV1、FVC和FEV1%均高于治疗前,差异有统计学意义(P<0.05),且观察组高于对照组,差异有统计学意义(P<0.05);两组mMRC评分和CAT评分均低于治疗前,差异有统计学意义(P<0.05),且观察组低于对照组,差异有统计学意义(P<0.05);两组呼气末膈肌厚度、吸气末膈肌厚度和膈肌活动度均高于治疗前,差异有统计学意义(P<0.05),且观察组高于对照组,差异有统计学意义(P<0.05)。结论低频体外膈肌起搏器联合呼吸功能锻炼者可通过增强膈肌功能改善AECOPD患者的临床症状和肺功能。 Objective To explore the effect of low-frequency external diaphragm pacemaker combined with respiratory function exercise on lung function and diaphragm function in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 84 patients with AECOPD who were treated in the respiratory department of the hospital from January to August 2022 were selected and randomly divided into two groups with 42 cases in each group.The patients in two groups were treated with basic treatment such as conventional drug therapy,artificial back knocking for expectoration,and continuous low-flow oxygen inhalation.The patients in control group was treated with routine respiratory function exercise,and the patients in experimental group was treated with low-frequency external diaphragmatic pacemaker on the basis of treatment in control group,and the patients in two groups were treated continuously for 4 weeks.The changes of pulmonary function indexes[forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC)and percentage of FEV1 to the predicted value(FEV1%pred)],clinical symptom indexes[modified medical research Council(mMRC)score and COPD assessment Test(CAT)score]and diaphragm function index[end-expiratory diaphragm thickness,end-inspiration diaphragm thickness and diaphragm activity]were compared.Results After treatment,FEV1,FVC and FEV1%in two groups were higher than those before treatment(P<0.05),and the experimental group was higher than that in control group(P<0.05);the mMRC scores and CAT scores in two groups were lower than those before treatment(P<0.05),and the test group was lower than that in control group(P<0.05);The end-expiratory diaphragm thickness,end-inspiration diaphragm thickness and diaphragm activity in two groups were higher than those before treatment(P<0.05),and the experimental group was higher than that in control group(P<0.05).Conclusion Low-frequency external diaphragm pacemaker combined with respiratory function exercise can improve the clinical symptoms and lung function of patients with AECOPD by enhancing diaphragm function.
