尼可地尔联合水化预防冠脉介入术后对比剂肾病的临床应用及其对血清胱抑素C、肌酐的影响

摘要 目的探讨尼可地尔联合水化预防冠脉介入术后对比剂肾病(contrast-induced nephropathy,CIN)的临床应用。方法选取2021年1月—2023年4月在新沂市人民医院行冠脉介入术治疗的80例冠状动脉粥样硬化性心脏病(简称冠心病)患者为研究对象,根据随机数表法分成两组,每组40例,观察组使用尼可地尔联合标准水化方案预防CIN,对照组使用标准水化方案预防CIN。比较两组胱抑素C(cystatin C,Cys-C)、肌酐(serum creatinine,Scr)等肾功能相关指标及乳酸脱氢酶(lactic dehydrogenase,LDH)、肌酸激酶同工酶(creatine kinase-MB,CK-MB)等心肌损害标志物及其他炎症因子的水平差异,比较两组CIN发生率及主要不良心血管事件(major adverse cardiac events,MACE)的发生情况。结果治疗后,观察组的血清Cys-C、Scr、LDH、CK-MB及炎症因子水平均低于对照组,差异有统计学意义(P<0.05)。观察组无CIN发生,对照组CIN发生率为15.00%,差异有统计学意义(χ^(2)=4.505,P=0.034)。结论在冠脉介入术后使用尼可地尔联合水化可有效减少造影剂相关肾损伤,改善循环功能。 Objective To explore the clinical application of nicorandil combined with hydration to prevent contrast-induced nephropathy(CIN)after coronary intervention.Methods 80 patients with coronary atherosclerotic heart disease who underwent coronary intervention in Xinyi People′s Hospital from January 2021 to April 2023 were selected as the study objects and divided into two groups of 40 cases each according to the randomized numerical table method.The observation group used nicorandil combined with the standard hydration regiment to prevent CIN,and the control group used the standard hydration regimen to prevent CIN.Differences in the levels of renal function-related indexes such as cystatin C(cys-C),serum creatinine(Scr),and markers of myocardial damage such as lactic dehydrogenase(LDH),creatine kinase-MB(CK-MB),and other inflammatory factors were compared between the two groups.The incidence of CIN and major adverse cardiac events(MACE)were compared between the two groups.Results The serum Cys-C,Scr,LDH,CK-MB and inflammatory factor levels in the observation group were lower than those in the control group after treatment,and the difference was statistically significant(P<0.05).No CIN occurred in the observation group,and the incidence of CIN of the control group was 15.00%,and the difference was statistically significant(χ^(2)=4.505,P=0.034).Conclusion The use of nicorandil combined with hydration after coronary intervention can effectively reduce contrast-associated renal injury and improve circulatory function.
