健康女性与卵巢子宫内膜异位囊肿患者阴道菌群研究

A study on vaginal microbiota of healthy women and patients with ovarian endometriomas

摘要 目的探讨卵巢子宫内膜异位囊肿患者与健康女性阴道菌群的差异。方法选取2022年5月至2022年12月北京大学深圳医院诊治的卵巢子宫内膜异位囊肿患者24例为研究组,选择体检人群中健康女性21例为对照组,采集穹隆部分泌物,采用16S rRNA基因扩增子测序技术对样本内原核细菌的16S rRNA进行高变区V4~V5区域测序,通过生物信息学方法对测序结果进行分析。结果研究组和对照组阴道菌群α、β多样性比较,差异无统计学意义(P>0.05),以乳酸杆菌为优势菌,其中在研究组中占87.5%(21/24),在对照组中占90.48%(19/21);研究组中放线菌门和Fannyhessea vaginae增高;在区分健康和疾病人群特征重要性最强的前15种菌种中,咽峡炎链球菌、柯气球菌、解脲支原体、Veillonella montpellierensis、Fannyhessea vaginae、Sneathia sanguinegens、无乳链球菌7种在研究组阴道菌群中相对丰度显著增加(P<0.05)。结论卵巢子宫内膜异位囊肿患者阴道菌群中生殖道炎症相关的细菌增多,可能参与疾病的发生发展。 Objective To investigate the differences in vaginal microbiota between healthy and ovarian endometriomas women.Methods 24 patients with ovarian endometriomas treated at Peking University Shenzhen Hospital from May 2022 to December 2022 were enrolled in the study group,and 21 healthy women were selected as control group.Vagina fornix samples were collected and the 16S ribosomal RNA of the prokaryotic bacteria in the sample was sequenced in high-variant V4~V5 region by 16S rRNA amplicon sequencing technology.The sequencing results were analyzed by bioinformatics methods.Results There was no statistical difference in alpha and beta diversity between the study and control groups(P>0.05),and were dominated by Lactobacillus[87.5%(21/24)in study group and 90.48%(19/21)in control group].Actinobacteria and Fannyhessea vaginae were increased in study group.The relative abundance of seven species,Streptococcus anginosus,Aerococcus christensenii,Ureaplasma urealyticum,Veillonella montpellierensis,Fannyhessea vaginae,Sneathia sanguinegens and Streptococcus agalactiae,were higher in ovarian endometriomas group(P<0.05).Conclusion Inflammation-associated bacteria are increased in ovarian endometriomas and may play a role in the development of the disease.
