恶性肺磨玻璃结节患者经HRCT检查的影像学参数及诊断效能分析 被引量:1

摘要 目的:分析恶性肺磨玻璃结节患者经高分辨率CT(HRCT)检查的影像学参数及诊断效能。方法:选取2021年1月至2023年1月广州市番禺区中医院60例疑似恶性肺磨玻璃结节患者为研究对象,纳入患者均接受常规CT检查与HRCT检查,并对其诊断结果及阳性征象检出率进行分析比较;对比阳性患者、阴性患者HRCT影像学参数。结果:经病理检查确诊阳性、阴性例数分别为40例、20例,常规CT检查确诊阳性、阴性例数分别为32例、28例;HRCT确诊阳性、阴性例数分别为39例、21例。HRCT检查诊断准确度、灵敏度及特异度(98.33%、97.50%、100.00%)相较于常规CT检查(70.00%、67.50%、75.00%)较高(P<0.05);阳性患者HRCT影像学参数(形态、分叶、毛刺、空泡征、空气支气管征、胸膜牵拉征、结节大小、平均CT值)对比差异显著(P<0.05)。结论:HRCT在恶性肺磨玻璃结节中具有较高的诊断效能,其取得的相关影像参数(形态、分叶、毛刺、空泡征、空气支气管征、胸膜牵拉征、结节大小、平均CT值)均可作为恶性肺磨玻璃结节的诊断依据。 Objective:To analyze the imaging parameters and diagnostic efficacy of high-resolution CT(HRCT)examination in patients with malignant pulmonary ground glass nodules.Methods:60 patients with suspected malignant pulmonary ground glass nodules in Panyu District Hospital of Traditional Chinese Medicine from January 2021 to January 2023 were selected as the research subjects.All included patients underwent routine CT examination and HRCT examination,and their diagnostic results and positive sign detection rates were analyzed and compared;additionally,compare HRCT imaging parameters between positive and negative patients.Results:The number of confirmed positive and negative cases through pathological examination was 40 and 20,respectively.The number of confirmed positive and negative cases through routine CT examination was 32 and 28,respectively.The number of confirmed positive and negative cases through HRCT examination was 39 and 21,respectively.The diagnostic accuracy,sensitivity,and specificity of HRCT examination(98.33%,97.50%,100.00%)were higher than that of routine CT examination(70.00%,67.50%,75.00%),(P<0.05);there were significant differences in HRCT imaging parameters(morphology,lobulation,spicules,vacuole sign,air bronchogram sign,pleural traction sign,nodule size,average CT value)among positive patients(P<0.05).Conclusion:HRCT has high diagnostic efficacy in analyzing malignant pulmonary ground glass nodules,and the relevant imaging parameters obtained(morphology,lobulation,spicules,vacuole sign,air bronchogram sign,pleural traction sign,nodule size,average CT value)can serve as the diagnostic basis for malignant pulmonary ground glass nodules.
