摘要
目的:探讨不同频率电针对前交叉韧带损伤家兔脊髓组织突触相关蛋白及神经递质的影响。方法:24只健康新西兰兔随机分为空白组、模型组、低频电针组、高频电针组,每组6只。模型组、低频电针组、高频电针组建立兔膝关节前交叉韧带损伤模型。造模后第7天低频电针组、高频电针组取患侧血海、梁丘行不同频率电针治疗,其余各组仅抓取、固定,不进行电针干预。连续干预21 d,1次/d。造模后第7天、干预结束后采用纤维丝测痛仪测量各组家兔的痛阈值;干预结束后ELISA检测背根神经节中的前列腺素E2(PGE2)、P物质(SP)、谷氨酸(Glu)、γ-氨基丁酸(GABA)水平,蛋白印迹法检测背根神经节中突触素(SYN)、突触后致密蛋白95(PSD95)的表达水平。结果:与空白组比较,模型组、低频电针组、高频电针组家兔的机械痛阈值降低(均P<0.01);干预后,与模型组比较,低频电针组、高频电针组家兔的机械痛阈值明显上升(均P<0.01),低频电针组改善优于高频电针组。低频电针组、高频电针组家兔背根神经节中的PGE2、SP、Glu表达较模型组降低,低频电针组低于高频电针组(均P<0.05);低频电针组、高频电针组家兔背根神经节中的GABA表达较模型组升高,低频电针组高于高频电针组(均P<0.05)。低频电针组、高频电针组家兔背根神经节中的SYN、PSD95表达较模型组降低,低频电针组低于高频电针组(均P<0.05)。结论:不同频率电针干预前交叉韧带损伤家兔具有良好的镇痛效应,其机制可能与减少疼痛介质及兴奋性神经递质表达,上调抑制性神经递质表达,下调SYN、PSD95突触相关蛋白表达,调节脊髓突触可塑性,从而缓解脊髓节段中枢敏化相关。
Objective:To investigate the effects of different frequency of electricity on synapse-related proteins and neurotransmitters in the spinal cord of rabbits with anterior cruciate ligament injury.Methods:Twenty-four healthy New Zealand rabbits were randomly divided into blank group,model group,low-frequency electroacupuncture group and high-frequency electroacupuncture group,with six rabbits in each group.The model group,low-frequency electroacupuncture group and high-frequency electroacupuncture group were established as rabbit knee ACL injury models.The Xuehai and Liangqiu on the affected side were treated with different frequencies electroacupuncture in the low-frequency electroacupuncture group and high-frequency electroacupuncture group on the 7th day after modeling,and the other groups were only captured and fixed without electroacupuncture intervention.The intervention was carried out continuously for 21 d,once a day.The pain thresholds of rabbits in each group were measured by fiber wire pain meter on the 7th day after modeling and at the end of the intervention.PGE2,SP,Glu and GABA in DRG were detected by ELISA at the end of the intervention,and the expression levels of SYN and PSD95 proteins in DRG were detected by Western blot.Results:The mechanical pain thresholds of rabbits in the model group,low-frequency electroacupuncture group and high-frequency electroacupuncture group decreased compared with the blank group(all P<0.01).After the intervention,the mechanical pain thresholds of rabbits in the low-frequency electroacupuncture group and high-frequency electroacupuncture group increased significantly compared with the model group(all P<0.01),and the improvement in the low-frequency electroacupuncture group was superior to that of the high-frequency electroacupuncture group.The expressions of PGE2,SP and Glu in low-frequency electroacupuncture group and high-frequency electroacupuncture group were lower than those in model group,and the low-frequency electroacupuncture group lower than those in high-frequency electroacupuncture group(all P<0.05).The expression of GABA in dorsal root ganglia of rabbits in low-frequency electroacupuncture group and high-frequency electroacupuncture group was higher than that in model group(all P<0.05).The expression of SYN and PSD95 in the DRG of rabbits in the low-frequency electroacupuncture group and the high-frequency electroacupuncture group were both reduced compared with that of the model group,and the low-frequency electroacupuncture group was lower than that of the high-frequency electroacupuncture group(all P<0.05).Conclusion:Different frequency electroacupuncture interventions in rabbits with ACL injury have good analgesic effects,and the mechanism may be related to reducing the expression of pain mediators and excitatory neurotransmitters,up-regulating the expression of inhibitory neurotransmitters,down-regulating the expression of synaptic-related proteins of SYN and PSD95,which regulate the synaptic plasticity of the spinal cord,thus alleviating the sensitization of spinal cord segments in the center of the spinal cord.
