摘要
目的 探讨床旁血滤对治疗重症急性胰腺炎的临床效果。方法 选取2020年7月-2022年8月本院接诊的66例重症急性胰腺炎患者,对其有关临床资料展开回顾性分析,并划分为两个对比组。血滤组33例,应用床旁血滤治疗;常规组33例,应用常规方式治疗。比较血滤组与常规组治疗重症急性胰腺炎的临床效果。结果 血滤组治疗效果比常规组强,对比差异有统计学意义(P<0.05)。血滤组症状改善时间比常规组短,对比差异有统计学意义(P<0.05)。治疗前血滤组与常规组重症急性胰腺炎患者的临床指标接近(P>0.05);治疗后两组患者的临床指标均有改善,且血滤组的APACHE-Ⅱ评分、IL-6、CRP、血清TG水平比常规组低,血氧分压比常规组高,对比差异有统计学意义(P<0.05)。治疗前血滤组与常规组的收缩压、心率、呼吸频率接近(P>0.05);治疗后两组患者的收缩压、心率、呼吸频率均有改善,且血滤组比常规组的收缩压、心率、呼吸频率理想,对比差异有统计学意义(P<0.05)。结论 床旁血滤对治疗重症急性胰腺炎的临床效果更理想,值得临床广泛运用。
Objective To explore clinical effect of bedside blood filtration in treatment of severe acute pancreatitis.Methods The paper chose 66 patients with severe acute pancreatitis from July 2020 to August 2022,reviewed and analyzed relevant clinical data and divided them into two comparative groups.33 cases in hemofiltration group was treated with bedside hemofiltration treatment,while 33 cases in routine group with routine treatment.Clinical effect of blood filtration group and routine group was compared.Results Treatment effect of hemofiltration group was stronger than routine group with severe acute pancreatitis,with statistically significant difference(P<0.05).Symptom improvement time of severe acute pancreatitis patients in hemofiltration group was shorter than routine group,difference was statistically significant(P<0.05).Clinical indicators of severe acute pancreatitis patients in hemofiltration group and routine group were similar before treatment,(P>0.05).After treatment,clinical indicators of both groups improved,APACHE-Ⅱscores,IL-6,CRP and serum TG levels in hemofiltration group were lower than routine group,while blood oxygen partial pressure was higher,with statistically significant difference(P<0.05).Before treatment,systolic blood pressure,heart rate and respiratory rate in hemofiltration group and routine group was similar(P>0.05).After treatment,systolic blood pressure,heart rate and respiratory frequency of both groups improved and systolic blood pressure,heart rate and respiratory frequency of hemofiltration group was more ideal than routine group,with statistically significant difference(P<0.05).Conclusion Bedside blood filtration can ahcieve ideal clinical effect in treatment of severe acute pancreatitis,which is worthy of wide application in clinical practice.
