摘要
目的分析病毒性心肌炎外周血正五聚体蛋白-3(PTX-3)和心型脂肪酸结合蛋白(h-FABP)与A重复优势蛋白(GARP)表达及意义.方法选取2020年1月-2021年12月上海市同济医院收治的119例病毒性心肌炎患者为病例组,另随机选取80名健康体检者为对照组,病例组患者根据病情严重程度分为轻、中和重度,并随访12个月观察预后.收集各组一般资料,酶联免疫吸附法检测所有研究对象外周血PTX-3、h-FABP、GARP水平.结果与对照组相比,病例组PTX-3、h-FABP表达较高,GARP表达水平较低(P<0.05).病例组患者,与轻度相比,中、重度患者PTX-3、h-FABP表达较高,GARP表达水平较低(P<0.05),与中度相比,重度患者PTX-3、h-FABP表达较高,GARP表达水平较低(P<0.05);与预后良好相比,预后不良PTX-3、h-FABP表达较高,GARP表达水平较低(P<0.05).受试者工作特征(ROC)曲线显示,三项联合对病毒性心肌炎的诊断价值高于PTX-3、h-FABP、GARP单项检测(P<0.05).结论病毒性心肌炎患者外周血PTX-3、h-FABP表达水平升高,GARP表达水平降低,联合检测PTX-3、h-FABP、GARP表达水平,可用于病毒性心肌炎的诊断.
OBJECTIVE To analyze the expressions of peripheral blood N-pentameric protein-3(PTX-3),heart-type fatty acid binding protein(h-FABP)and A the repeat predominant protein(GARP)in the patients with viral myocarditis and analyze their significance.METHODS A total of 119 patients with viral myocarditis who were treated in Shanghai Tongji Hospital from Jan 2020 to Dec 2021 were assigned as the case group,meanwhile,80 healthy people who received physical examination were chosen as the control group.The patients of the case group were divided into the mild,moderate and severe groups according to the severity of illness.The treatment outcomes were observed after a 12-month follow-up.The baseline data were collected from the groups,and the levels of peripheral blood PTX-3,h-FABP and GARP were detected for all of the research subjects by enzyme-linked immunosorbent assay.RESULTS The expression levels of PTX-3 and h-FABP of the case group were higher than those of the control group,while the expression level of GARP of the case group was lower than that of the control group(P<0.05).In the case group,the expression levels of PTX-3 and h-FABP of the moderate,severe groups were higher than those of the mild group,while the expression level of GARP of the moderate,severe groups was lower than that of the mild group(P<0.05);the expression levels of PTX-3 and h-FABP of the severe group were higher than those of the moderate group,while the expression level of GARP of the severe group was lower than that of the moderate group(P<0.05).The expression levels of PTX-3 and h-FABP of the patients with poor prognosis were higher than those of the patients with favorable prognosis,while the expression level of GARP of the patients with poor prognosis was lower than that of the favorable prognosis(P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the value of the joint detection of the three indexes PTX-3,h-FABP and GARP was higher than that of the single detection in diagnosis of the viral myocarditis(P<0.05).CONCLUSION The patients with viral myocarditis show the rise of expression levels of peripheral blood PTX-3,h-FABP and the decline of expression level of GARP.The joint detection of PTX-3,h-FABP and GARP can be used for the diagnosis of viral myocarditis.
