摘要
黄元御“枢轴运动”理论阐释了中气之轴斡旋于内,带动肝、心、肺、肾四维之轮转动于外的循环生理观。凡病则虚,凡病则郁,本文基于“枢轴运动”理论,探析中气虚及四维郁作为始动因素引起机体阴阳不济、浊瘀内生进而导致肝癌的病机演变过程,提出戊土不降,阳不下蛰;己土不升,阴不上根;肝无升路,木陷血瘀;肺无降路,金逆浊留的虚郁致肝癌观,同时提出了培中气以复职升降,运四维以斡旋气机的肝癌治疗原则,以期使轴运轮行,气血阴阳自和,探索临床肝癌诊治的新思路。
HUANG Yuan-yu’s“pivot movement”theory explains the circulatory physiological view that the middle energizer Qi acts like the axle inside and the other four elements(liver,heart,lung and kidney)act like the wheels outside.All diseases are caused by deficiency and depression.Based on the theory of“pivot movement”,this paper discussed the pathogenesis evolution process of imbalance of yin and yang and the formation of dampness-turbidity and blood stasis in liver cancer caused by deficiency of middle Qi and depression of four elements(liver,heart,lung and kidney),which were the initial factors.It is proposed that the failure of stomach in descending brings about the failure of yang in hiding,the failure of spleen in ascending brings about the failure of yin in upward nourishing,the liver Qi fails in ascending and then results in blood stasis,the lung Qi fails in descending and then results in phlegm,which contribute to the deficiency and depression pathogenies of liver cancer.Meanwhile,the therapeutic principle for liver cancer is put forward,which is to replenish spleen and stomach and restore the rise and fall of the middle Qi,and promote the Qi movement through activating the four elements(liver,heart,lung and kidney),so as to make the axis carry the wheel,and harmonize the yin,yang,Qi and blood.The exploration of the view of liver cancer caused by deficiency and depression will provide a new idea for the clinical diagnosis and treatment of liver cancer.
