结肠癌患者转化生长因子β1和胰腺癌第4座位缺失基因的表达及其临床诊断价值分析

Expression of TGF-β1and DPC4in patients with colon cancer and its clinical diagnostic value

摘要 目的探究结肠癌患者转化生长因子β1(TGF-β1)和胰腺癌第4座位缺失基因(DPC4)的表达及其在临床诊断中的价值。方法选取2021-01-01-2022-05-30信阳市肿瘤医院治疗的80例结肠癌患者作为观察组,另选取同期85名健康体检者作为对照组。免疫组织化学法检测结肠癌患者癌组织及癌旁组织中TGF-β1和DPC4蛋白表达;酶联免疫吸附(ELISA)法检测所有受试者血清中DPC4和TGF-β1表达水平;结肠癌患者DPC4和TGF-β1表达水平相关性行Pearson分析;受试者工作特征(ROC)曲线分析TGF-β1和DPC4的表达水平对结肠癌的诊断价值。结果结肠癌组织中DPC4蛋白高表达率为31.45%(25/80),低于癌旁组织的70.00%(56/80),差异有统计学意义,χ^(2)=24.029,P<0.001;TGF-β1蛋白高表达率为65.00%(52/80),高于癌旁组织的23.75%(19/80),差异有统计学意义,χ^(2)=25.574,P<0.001。观察组患者血清DPC4表达水平为(33.56±10.52)pg/mL,低于对照组的(52.31±16.34)pg/mL,差异有统计学意义,t=8.704,P<0.001;血清TGF-β1表达水平为(38.89±12.35)ng/mL,高于对照组的(20.34±6.12)ng/mL,差异有统计学意义,t=12.334,P<0.001。肿瘤直径≥5cm、TNM分期为Ⅲ+Ⅳ期、发生淋巴结转移、低分化程度的结肠癌患者DPC4表达水平低于肿瘤直径<5cm、TNM分期为Ⅰ+Ⅱ期、无淋巴结转移、中/高分化程度的患者,差异有统计学意义,均P<0.05;TNM分期为Ⅲ+Ⅳ期、浸润深度侵及浆膜层、发生淋巴结转移的结肠癌患者TGF-β1表达水平高于TNM分期为Ⅰ+Ⅱ期、浸润深度未侵及浆膜层、无淋巴结转移的患者,差异有统计学意义,均P<0.05。结肠癌患者血清中DPC4与TGF-β1表达呈负相关,r=-0.430,P<0.05。DPC4和TGF-β1二者联合诊断结肠癌的曲线下面积(AUC)优于其各自单独诊断,均P<0.05。结论结肠癌患者DPC4表达显著降低,TGF-β1表达显著升高,二者具有负相关性,且与患者临床病理特征密切相关,两项指标联合诊断对预测结肠癌发生具有重要价值。 Objective To explore the expression of transforming growth factor-β1(TGF-β1)and deleted in pancreatic carcinoma locus 4(DPC4)in patients with colon cancer and its value in clinical diagnosis.Methods A total of 80patients with colon cancer treated by Xinyang Cancer Hospital from January 1,2021to May 30,2022were selected as the observation group,and 85healthy subjects in the same period were selected as the control group.The expression of TGF-β1 and DPC4protein in cancer tissues and adjacent tissues of colon cancer patients was determined by immunohistochemistry.The expression levels of DPC4and TGF-β1in serum of all subjects were detected by enzyme-linked immunosorbent assay(ELISA).The correlation between the expression level of DPC4and TGF-β1in colon cancer patients was analyzed by Pearson analysis.The diagnostic value of TGF-β1and DPC4expression levels for colon cancer was indicated by receiver operating characteristic(ROC)curve.Results The high expression rate of DPC4protein in colon cancer tissues was 31.45%(25/80),which was lower than that in adjacent tissues 70.00%(56/80),and the difference was statistically significant,χ^(2)=24.029,P<0.001;the high expression rate of TGF-β1protein was 65.00%(52/80),which was highe than that of the adjacent tissues 23.75%(19/80),and the difference was statistically significant,χ^(2)=25.574,P<0.001.Serum DPC4expression level in observation group was(33.56±10.52)pg/ml,lower than that in control group(52.31±16.34)pg/ml,and the difference was statistically significant,t=8.704,P<0.001;the expression level of TGF-β1in serum was(38.89±12.35)ng/ml,which was higher than that in control group(20.34±6.12)ng/ml,and the difference was statistically significant,t=12.334,P<0.001.The expression level of DPC4in colon cancer patients with tumor diameter≥5cm,TNM stageⅢ+Ⅳ,lymph node metastasis and low differentiation was lower than that in patients with tumor diameter<5cm,TNM stageⅠ+Ⅱ,no lymph node metastasis and middle/high differentiation,the difference was statistically significant,all P<0.05;the expression level of TGF-β1in colon cancer patients with TNM stageⅢ+Ⅳ,deep invasion of serous layer and lymph node metastasis was higher than that in patients with TNM stageⅠ+Ⅱ,no invasion of serous layer and no lymph node metastasis,and the difference was statistically significant,all P<0.05.The expression of TGF-β1in serum of patients with colon cancer was negatively correlated with DPC4,r=-0.430,P<0.05.The area under the curve(AUC)of DPC4and TGF combined in the diagnosis of colon cancer was better than their respective individual diagnoses,both P<0.05.Conclusions The expression of DPC4in patients with colon cancer is significantly decreased,and the expression of TGF-β1is significantly increased.The two have a negative correlation and are closely related to the clinical and pathological characteristics of patients.The combined diagnosis of the two indexes is of great value in predicting the occurrence of colon cancer.
