摘要
目的 观察关节镜联合中医小针刀治疗髌股骨关节炎的临床效果。方法 选取早期髌、股骨关节炎患者93例,分3组各31例,观察组(GJJ)31例,采用微创关节镜手术治疗,观察组(ZD)31例,采用小针刀技术治疗,联合组联合治疗。结果 治疗后30 d、治疗后3个月,3组Kujala髌股关节功能评分均高于治疗前(P<0.05)、 VAS疼痛评分均低于治疗前(P<0.05),且联合组Kujala髌股关节功能评分均高于观察组GJJ和观察组ZD(P<0.05)、VAS疼痛评分均低于观察组GJJ和观察组ZD(P<0.05);治疗后3个月TNF-α、IL-6炎症因子含量均低于治疗前(P<0.05),且联合组均低于观察组GJJ和观察组ZD(P<0.05)。结论 微创关节镜手术联合中医小针刀技术治疗早期的轻中度髌股骨关节炎效果好,能有效缓解疼痛等临床症状,同时能很好地抑制炎症反应,利于功能恢复。
Objective To observe the clinical effect of arthroscopy combined with acupotomy in the treatment of patellofemoral osteoarthritis.Methods Ninety-three patients with early patellofemoral arthritis were divided into 3 groups,with arthroscopy group(GJJ)of 31 cases was treated with minimally invasive arthroscopic surgery,acupotomy group(ZD)of 31 cases was treated with acupotomy technique,and combined group of 31 cases was treated with arthroscopy combined with acupotomy.Results 30 days after treatment and 3 months after treatment,Kujala patellofemoral joint function score was higher than before treatment(P<0.05),VAS pain score was lower than before treatment(P<0.05).The Kujala patellofemoral joint function score of the combined group was higher than that of the GJJ and ZD(P<0.05),and the VAS pain score was lower than that of the GJJ and ZD(P<0.05).The contents of TNF-αand IL-6 inflammatory factors at 3 months after treatment were lower than those before treatment(P<0.05),and the combined group were lower than the GJJ and ZD(P<0.05).Conclusion Arthroscopy combined with acupotomy is effective in treating mild to moderate patellofemoral osteoarthritis,it can effectively relieve pain and other clinical symptoms,and it can well inhibit the inflammatory response,which is conducive to functional recovery.
