摘要
目的探讨水胶体拉合免缝法联合银离子抗菌敷料在开腹手术后伤口愈合中的应用效果及对生长因子水平的影响。方法选取2020年1月至2022年1月我院收治的150例开腹手术患者为研究对象,根据伤口护理方法不同将其分为对照组和观察组,各75例。对照组采用常规缝合和换药护理,观察组采用水胶体拉合免缝法联合银离子抗菌敷料护理。比较两组的护理效果。结果观察组的单位时间内疼痛程度、单位时间内伤口愈合速度及新鲜肉芽组织形成时间优于对照组(P<0.05)。观察组的伤口愈合时间、总换药时间短于对照组,总换药次数少于对照组(P<0.05)。护理后,观察组的血管内皮生长因子(VEGF)、表皮生长因子(EGF)、碱性成纤维细胞生长因子(bFGF)水平高于对照组(P<0.05)。护理后,观察组的白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平低于对照组(P<0.05)。结论水胶体拉合免缝法联合银离子抗菌敷料在开腹手术后伤口愈合中的应用效果较好,能调节生长因子水平,下调炎性因子表达,值得推广。
Objective To investigate the application effect of hydrocolloid pull-up suture-free method combined with silver ion antibacterial dressing on wound healing after laparotomy and its influence on growth factor level.Methods A total of 150 patients undergoing laparotomy admitted in our hospital from January 2020 to January 2022 were selected as the research objects.According to different wound nursing methods,the patients were divided into control group and observation group,with 75 cases in each group.The control group was treated with routine suture and dressing change nursing,while the observation group was treated with hydrocolloid pull-up suture-free method combined with silver ion antibacterial dressing nursing.The nursing effects of the two groups were compared.Results The pain degree per unit time,wound healing rate per unit time and fresh granulation tissue formation time in the observation group were better than those in the control group(P<0.05).The wound healing time and total dressing change time in the observation group were shorter than those in the control group,and the total dressing change times was less than that in the control group(P<0.05).After nursing,the levels of vascular endothelial growth factor(VEGF),epidermal growth factor(EGF)and basic fibroblast growth factor(bFGF)in the observation group were higher than those in the control group(P<0.05).After nursing,the levels of interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group(P<0.05).Conclusion Hydrocolloid pull-up suture-free method combined with silver ion antibacterial dressing has a good application effect on wound healing after laparotomy,it can regulate the levels of growth factors and down-regulate the expression of inflammatory factors,which is worthy of promotion.
