摘要
目的分析乳腺黏液腺癌(MBC)的自动乳腺全容积成像(ABVS)冠状面图像和高频超声图像特征,提高对本病的认识。资料与方法回顾性分析延边大学附属医院2017年7月—2021年10月经术后病理证实的MBC患者28例,包括单纯型乳腺黏液腺癌(PMBC)和混合型乳腺黏液腺癌(MMBC)各14例,观察病灶ABVS冠状面形态、边缘特征及高频超声下形态、边缘、纵横比、内部及后方回声、血供情况等特征,并与超声特征进行比较。结果PMBC与MMBC组间高频超声声像图特征比较,MMBC组较PMBC组血流丰富,仅血流分级差异有统计学意义(χ^(2)=10.296,P<0.05),病灶形态、边缘、纵横比、内部及后方回声差异均无统计学意义(P均>0.05)。腺体层上,ABVS冠状面对PMBC、MMBC病灶的恶性征象诊出数量多于高频超声,两种检查显示病灶形态、边缘是否模糊及有无边缘成角差异有统计学意义(χ^(2)=17.742、14.998、14.703,P均<0.05),边缘毛刺差异无统计学意义(P>0.05);ABVS冠状面见病灶后方出现白墙征(25/28,89.3%)。脂肪层上,高频超声中形态较规则的肿块在ABVS冠状面上边缘较光整,周边见强回声晕包绕(19/28,67.9%)。结论MBC高频超声图像具有比较典型的超声特征;ABVS冠状面可以完整观察病灶的形态特征,乳腺病灶的恶性征象表现优于高频超声,二者联合有助于更好地显示MBC的超声特征,为本病的早期临床干预提供参考依据。
Purpose To analyze the image characteristics of mucinous breast carcinoma(MBC)via automated breast volume scanner(ABVS)coronal plane and high-frequency ultrasound and to improve the understanding of this disease.Materials and Methods A total of 28 cases pathologically confirmed MBC in the Affiliated Hospital of Yanbian University from July 2017 to October 2021 were retrospectively analyzed.All patients included 14 patients with pure mucinous breast carcinoma(PMBC)and 14 patients with mixed mucinous breast carcinoma(MMBC).The coronal morphology and edge characteristics of ABVS,morphology,edge,aspect ratio,internal echo,posterior echo,blood supply of high-frequency ultrasound were observed.Results There was significant difference in the blood flow classification between PMBC group and MMBC group by high-frequency ultrasound,and the blood flow of MMBC group was significantly richer than that of PMBC group(χ^(2)=10.296,P<0.05).There were no statistically significant differences in the shape,edge,aspect ratio,internal and posterior echo of the lesions between the two groups(all P>0.05).On the glandular layer,the number of malignant signs of PMBC and MMBC lesions on the coronal plane of ABVS was significantly more than those of high-frequency ultrasound.There was statistical significance in the shape of the lesions and the angulation of the edges between the two examination methods(χ^(2)=17.742,14.998,14.703,all P<0.05),and there was no statistical significance in the blurring and burring of the edges(P>0.05).The“white wall sign”(25/28,89.3%)often appeared behind the lesions on the coronal plane of ABVS.On the adipose layer,the mass with regular shape in high-frequency ultrasound had smooth edge on the ABVS coronal plane,surrounded by strong echo halo(19/28,67.9%).Conclusion High frequency ultrasound images have typical ultrasonic features of MBC.ABVS coronal plane can completely show the morphological characteristics of the lesions,and the performance of breast malignant signs may be better than that of high-frequency ultrasound.The combined diagnosis of the two methods is helpful to better display the ultrasonic characteristics of MBC and to provide a reference basis for the early clinical intervention.
