摘要
目的:探究血小板(platelet,PLT)、异型淋巴细胞对肾综合征出血热(hemorrhagic fever with renal syndrome,HFRS)的诊断价值。方法:选择2019年8月—2022年8月厦门大学附属第一医院收治的75例疑似HFRS患者作为研究对象,记录其体征表现,对其开展血常规检验明确PLT水平,并开展异型淋巴细胞检测明确此类细胞所占比例,以免疫球蛋白M(IgM)抗体检测结果为金标准,评估PLT、异型淋巴细胞及PLT联合异型淋巴细胞对HFRS的诊断价值。结果:HFRS患者典型症状为发热,研究对象中出现此症状者占比100%。PLT联合异型淋巴细胞对HFRS的阴性预测值、敏感度及诊断符合率高于PLT及异型淋巴细胞,差异有统计学意义(P<0.05);但PLT、异型淋巴细胞及PLT联合异型淋巴细胞诊断的阳性预测值、特异度比较,差异无统计学意义(P>0.05)。结论:PLT联合异型淋巴细胞可提高HFRS诊断准确率,为临床提供除体征以外的细胞学诊断依据。
Objective:To explore the diagnostic value of platelet(PLT)and atypical lymphocytes in hemorrhagic fever with renal syndrome(HFRS).Method:A total of 75 patients with suspected HFRS who admitted to the First Affiliated Hospital of Xiamen University from August 2019 to August 2022 were selected as the study subjects,their physical signs and manifestations were observed,and blood routine tests were conducted to determine their PLT levels,and the proportion of such cells was determined by the detection of atypical lymphocytes.Immunoglobulin M(IgM)antibody detection results were used as the gold standard,the diagnostic value of PLT,atypical lymphocytes and PLT combined with atypical lymphocytes in HFRS were evaluated.Result:The typical symptom of HFRS patients was fever,which accounted for 100%of subjects.The negative predictive value,sensitivity and diagnostic coincidence rate of PLT combined with atypical lymphocytes for HFRS were higher than those of PLT and atypical lymphocytes,and the differences were statistically significant(P<0.05);however,there were no significant differences in the positive predictive value and specificity of PLT,atypical lymphocytes and PLT combined with atypical lymphocytes(P>0.05).Conclusion:PLT combined with atypical lymphocytes can improve the diagnostic accuracy of HFRS,and provide cytological diagnosis basis other than signs for clinical practice.
