摘要
目的探索脐针联合中药热奄包对促进全身麻醉(简称“全麻”)下行胸腔镜手术患者术后胃肠道功能恢复的效果。方法按照随机数字表法将2022年4月至2023年3月在广州中医药大学顺德医院行全麻下胸腔镜手术的60例患者分为观察组(30例)和对照组(30例)。对照组在术后接受常规护理,观察组在术后接受脐针联合中药热奄包干预。对比2组的术后临床症状缓解情况等指标。结果观察组的临床症状缓解总有效率高于对照组,术后肠鸣音恢复时间、首次肛门排气时间和首次排便时间均早于对照组,住院时间较对照组短(P均<0.05)。观察组在术后12 h、24 h、48 h和72 h的腹胀评分均低于同期对照组(P均<0.05)。术后1 d,观察组健康调查量表36(SF-36)中生理功能、生理职能、躯体疼痛、总体健康和社会功能5个维度的得分均较对照组更高(P均<0.05);术后3 d,除精力和情感职能2个维度外,观察组其余维度的得分均高于对照组(P均<0.05);术后7 d,观察组的SF-36所有维度得分均高于对照组(P均<0.05)。结论脐针与中药热奄包联合干预能有效促进全麻下行胸腔镜手术患者术后胃肠道功能的恢复和临床症状的缓解,改善其术后生活质量。
Objective To explore the effect of umbilical acupuncture combined with traditional Chinese medicine hot package on the gastrointestinal function recovery after thoracoscopic surgery under general anesthesia.Methods Sixty patients undergoing thoracoscopic surgery under general anesthesia at Shunde Hospital of Guangzhou University of Chinese Medicine from April 2022 to March 2023 were divided into an observation group(30 cases)and a control group(30 cases)through random number table method.The control group received routine nursing after surgery,and the observation group received intervention of umbilical acupuncture combined with traditional Chinese medicine hot package after surgery.The postoperative symptom relief and other indicators were compared between the two groups.Results The total effective rate of symptom relief in the observation group was higher than that in the control group(P<0.05).The recovery time of postoperative bowel sound,first exhaust time,and first defecation time were earlier than those in the control group,and the length of hospitalization was shorter than that in the control group(all P<0.05).The abdominal distension scores of the observation group were lower than those of the control group at 12 h,24 h,48 h,and 72 h after surgery(all P<0.05).On the 1st day after surgery,the scores of role limitations due to physical health problems,physical functioning,bodily pain,general health,and social function of the Short Form-36 Health Survey(SF-36)in the observation group were higher than those in the control group(all P<0.05).On the 3rd day after surgery,the scores of all dimensions in the observation group were higher than those in the control group except for the scores of energy and role limitations due to emotional problems(all P<0.05).On the 7th day after surgery,the scores of all dimensions of the SF-36 in the observation group were higher than those in the control group(all P<0.05).Conclusion Umbilical acupuncture combined with traditional Chinese medicine hot package can effectively promote the recovery of gastrointestinal function and the relief of clinical symptoms in patients undergoing thoracoscopic surgery under general anesthesia,and improve their postoperative quality of life.
