铁死亡相关基因KIF20A对肾上腺皮质癌患者预后的影响

摘要 目的分析铁死亡相关基因对肾上腺皮质癌(ACC)患者的影响,并建立预后预测模型。方法使用TCGA数据库和FerrDb V2数据库综合分析得到ACC进展的关键铁死亡基因驱动蛋白超家族20A(kinesin family member 20A,KIF20A)。使用TCGA数据库、GTEx数据库和GEO数据库比较ACC和正常组织中KIF20A的表达情况。从TCGA数据库中下载ACC RNA-seq Counts数据及临床数据。基于KIF20A受试者工作特征曲线(receiver operating characteristic curve,ROC)的最佳截断值将其分为高表达组和低表达组,进行Kaplan-Meier生存分析并使用卡方检验探索其与临床指标的相关性。将KIF20A基因表达量与各个临床指标进行单因素和多因素COX回归筛选患者预后的独立影响因素。联合各项患者预后指标绘制列线图。结果TCGA数据库、GSE33371数据集和GSE14922数据集显示,KIF20A在ACC中的表达量明显高于正常组织(P<0.05);随着Stage分期的升高,KIF20A的表达量逐渐升高(P<0.05)。生存分析结果显示,KIF20A高表达组的ACC患者预后明显差于KIF20A低表达组。卡方检验显示,患者的M分期、N分期、T分期、Stage分期和化疗是影响KIF20A表达的因素(P<0.05)。Logistic分析显示,M分期和T分期是影响KIF20A表达的独立危险因素(P<0.05)。单因素COX回归显示,KIF20A、M分期、T分期、Stage分期和化疗为ACC患者预后的影响因素(P<0.05);多因素COX回归显示,KIF20A为ACC患者预后的独立影响因素(P<0.05)。联合患者临床因素如年龄、性别和肿瘤分期等构建了预后评估的列线图模型。结论铁死亡相关基因KIF20A是ACC患者预后的独立影响因素,由KIF20A和患者临床指标构建的列线图可精准预测患者的生存率。 Objective Analyzing the impact of ferroptosis-related genes on patients with Adrenocortical Carcinoma(ACC)and establishing a prognostic prediction model.Methods Comprehensive analysis of key ferroptosis-related genes driving the progression of ACC using TCGA database and FerrDb V2 database,focusing on Kinesin Family Member 20A(KIF20A).Comparing the expression of KIF20A in ACC and normal tissues using TCGA database,GTEx database,and GEO database.Downloading ACC RNA-seq Counts data and clinical data from TCGA database.Using the optimal cutoff value based on the Receiver Operating Characteristic curve(ROC)of KIF20A to divide them into high-expression and low-expression groups,conducting Kaplan-Meier survival analysis,and exploring its correlation with clinical indicators using chi-square tests.Using single-factor and multi-factor Cox regression to screen for independent prognostic factors for patients by comparing KIF20A gene expression with various clinical indicators.Creating nomogram to visualize the combined prognostic indicators for patients.Results TCGA database,GSE33371 dataset,and GSE14922 dataset demonstrate significantly higher expression of KIF20A in ACC compared to normal tissues(P<0.05).Additionally,KIF20A expression increases gradually with the advancement of Stage(P<0.05).Survival analysis results indicate that ACC patients with high KIF20A expression have significantly worse prognosis than those with low expression.Chi-square tests reveal that patients'M stage,N stage,T stage,Stage,and chemotherapy are factors affecting KIF20A expression(P<0.05).Logistic analysis further demonstrates that M stage and T stage are independent risk factors for KIF20A expression(P<0.05).Single-factor Cox regression indicates that KIF20A,M stage,T stage,Stage,and chemotherapy are prognostic factors for ACC patients(P<0.05).Multi-factor Cox regression demonstrates that KIF20A is an independent prognostic factor for ACC patients(P<0.05).A nomogram model for prognostic assessment was constructed by integrating clinical factors such as age,gender,and tumor stage.Conclusion Ferroptosis-related gene KIF20A is an independent prognostic factor for ACC patients,and the nomogram constructed from KIF20A and patient clinical indicators can accurately predict patient survival.
