摘要
目的分析性激素检测在不孕症患者临床诊断中的应用价值。方法选取2020年1月-2022年12月90例不孕症患者作为观察组,并选取同时间段90例健康体检妇女作为对照组,两组均行性激素检查,比较两组检查结果的差异,同时根据性激素检查结果,评价各项性激素指标诊断的灵敏性、特异性、准确性、阳性预测值、阴性预测值。结果①卵泡期,观察组促黄体生成素、促卵泡激素、垂体泌乳素、孕酮、睾酮高于对照组,雌二醇低于对照组,差异有统计学意义(P<0.05);排卵期,观察组促黄体生成素、促卵泡激素、垂体泌乳素、孕酮、睾酮、雌二醇低于对照组,差异有统计学意义(P<0.05);黄体期,观察组促黄体生成素、促卵泡激素高于对照组,垂体泌乳素、孕酮、睾酮、雌二醇低于对照组,差异有统计学意义(P<0.05);②性激素六项联合诊断的灵敏性、特异性、准确性、阳性预测值、阴性预测值高于单独的促黄体生成素检测、促卵泡激素检测、垂体泌乳素检测、孕酮检测、睾酮检测、雌二醇检测,差异有统计学意义(P<0.05)。结论性激素六项联合检测在不孕症患者中有着较高的灵敏性和特异性,可推广使用。
Objective Analyze the application value of sex hormone testing in the clinical diagnosis of infertility patients.Method 90 infertile patients from January 2020 to December 2022 were selected as the observation group,and 90 healthy women who underwent physical examinations during the same period were selected as the control group.Both groups underwent sex hormone tests to compare the differences in the test results between the two groups.At the same time,based on the sex hormone test results,the sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of various sex hormone indicators were evaluated for diagnosis.Result①During the follicular phase,the levels of luteinizing hormone,follicle stimulating hormone,pituitary prolactin,progesterone,and testosterone in the observation group were higher than those in the control group,while estradiol was lower than those in the control group,with significant differences(P<0.05);during ovulation,the levels of luteinizing hormone,follicle stimulating hormone,pituitary prolactin,progesterone,testosterone,and estradiol in the observation group were significantly lower than those in the control group(P<0.05);during the luteal phase,the levels of luteinizing hormone and follicle stimulating hormone in the observation group were higher than those in the control group,while pituitary prolactin,progesterone,testosterone,and estradiol were lower than those in the control group,with significant differences(P<0.05);②The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of the combined diagnosis of six sexual hormones were higher than those of luteinizing hormone test,follicle stimulating hormone test,pituitary prolactin test,progesterone test,testosterone test,and estradiol test alone,with significant differences(P<0.05).Conclusion The combined detection of six sexual hormones has high sensitivity and specificity in infertility patients,and can be widely used.
