摘要
目的 探析肱骨近端骨折髓内支撑术后康复治疗中,配合中医针刺治疗,在术后患者肱骨及肩关节的功能恢复中的作用。方法 选择2021年9月—2022年3月北部战区总医院收治的60例肱骨近端骨折患者作为研究对象,使用随机数字表法将患者分为观察组和对照组,两组患者均行肱骨近端骨折髓内支撑术治疗,对照组患者在术后进行常规康复治疗,观察组患者在常规康复治疗基础上联合中医针刺治疗。结果 两组术后1、3个月VAS评分与术前比较,差异均有显著性(P<0.05);观察组术后1、3个月VAS评分均低于对照组,差异有显著性(P<0.05)。术后6个月的QOL评分,与治疗前比较,评分均明显升高,差异显著(P<0.05),且两组患者术后6个月的QOL评分均较治疗前升高,组间差异显著(P<0.05)。对两组患者Constant-Murley评分进行比较,术后1个月、3个月、6个月肩关节外旋、前屈、外展、内旋的评分与术前相比,均明显改善(P<0.05);术后1个月时,两组患者组间差异显著(P<0.05)。两组患者治疗后的Neer评分与治疗前相比,差异显著(P<0.05),两组患者术后6个月Neer评分组间比较,与同期对照组相比,观察组患者Neer评分显著增加(P<0.05)。结论 肱骨近端骨折髓内支撑术后配合中医针刺治疗可以促进肩关节功能恢复,为提高生活质量,减轻术后疼痛。
Objective To explore the functional recovery of humerus and shoulder joint by traditional Chinese medicine acupuncture on the basis of postoperative rehabilitation of proximal humerus fracture with intramedullary support.Methods From September 2021 to March 2022,60 patients with proximal humeral fractures in the Northern Theater Command General Hospital were admitted in the research.A random number table method was used to divide the patients into observation group and control group.Patients in both groups were treated with intramedullary support for proximal humerus fractures.Patients in the control group received conventional rehabilitation treatment after surgery,while patients in the observation group were treated with traditional Chinese medicine acupuncture on the basis of conventional rehabilitation treatment.Results In both groups,the VAS scores of 1 and 3 months after operation were significantly different from those before operation(P<0.05).The VAS score of the observation group was lower than that of the control group at 1 and 3 months after operation(P<0.05).The QOL score at 6 months after operation was significantly higher than that before treatment(P<0.05),and the QOL score of the two groups at 6 months after operation was higher than that before treatment(P<0.05).Comparison of Constant-Murley score between the two groups showed that external rotation,forward flexion,abduction and internal rotation of shoulder were significantly improved at 1 month,3 months and 6 months after operation compared with those before operation(P<0.05).At 1 month after operation,the difference between the two groups was significant(P<0.05).The Neer score of the two groups after treatment was significantly different from that before treatment(P<0.05).Compared with the control group at the same time,the neer score of the patients in the observation group increased significantly(P<0.05) 6 months after operation.Conclusion Intramedullary support combined with traditional Chinese medicine acupuncture treatment for proximal humerus fracture can promote the recovery of shoulder joint function,which is helpful in improving the quality of life and reducing the postoperative pain of patients.
