摘要
目的 分析绝经后子宫脱垂患者采用腹腔镜圆韧带阴道残端悬吊联合阴式子宫切除治疗的临床效果。方法 收集在徐州市第六人民医院接受治疗的68例绝经后子宫脱垂患者(2019年1月~2022年12月)资料,依据随机数字表法分为对照组(34例,阴式子宫切除+阴道前壁修补术治疗)、观察组(34例,腹腔镜圆韧带阴道残端悬吊+阴式子宫切除治疗)。对比两组手术指标、并发症发生情况及盆底肌肌力情况。结果 观察组手术时间为(116.25±21.34)min较对照组的(72.75±12.35)min更长;出血量为(98.57±16.54)ml较对照组的(84.56±13.32)ml更多(P<0.05);两组患者排气、住院时间对比无显著差异(P>0.05)。观察组和对照组并发症发生率为20.59%、23.53%,无显著差异(P>0.05)。观察组患者盆底肌肌力分级优于对照组(P<0.05)。结论 予以绝经后子宫脱垂患者腹腔镜圆韧带阴道残端悬吊联合阴式子宫切除治疗与单纯阴式子宫切除治疗均可发挥良好疗效,且安全性较高,但前者可更好的改善患者盆底肌肌力。
Objective To analyze the clinical effect of laparoscopic round ligament with vaginal stump suspension combined with vaginal hysterectomy in postmenopausal patients with uterine prolapse.Methods 68 patients with postmenopausal uterine prolapse who were treated in the Sixth People's Hospital of Xuzhou City(January 2019 December 2022)were randomly divided into the control group(34 cases,vaginal hysterectomy+vaginal anterior wall repair)and the observation group(34 cases,abdominal endoscope round ligament vaginal stump suspension+vaginal hysterectomy).The surgical indicators,incidence of complications and the pelvic floor muscle strength were compared between the two groups.Results The operative time of(116.25±21.34)min in the observation group was longer than that of(72.75±12.35)min in the control group,and the amount of blood loss of(98.57±16.54)ml was longer than that of(84.56±13.32)ml in the control group(P<0.05).There was no significant difference between the two groups in terms of exhaust and hospitalization time(P>0.05).There was no significant difference in the incidence of complications between the two groups(20.59%vs 23.53%)(P>0.05).The pelvic floor muscle strength of the observation group was better than the control group(P<0.05).Conclusion In postmenopausal patients with uterine prolapse,both laparoscopic round ligament vaginal stump suspension combined with vaginal hysterectomy or simple vaginal hysterectomy have good efficacy and high safety,but the former can better improve the pelvic floor muscle strength of patients.
