摘要
目的:探讨四维彩超联合颈项透明层厚度(NT)超声检查在产前胎儿畸形筛查中的应用价值。方法:选取2021年1月—2023年6月于东营市东营区人民医院行产前胎儿畸形筛查的1415例孕妇作为研究对象,均于孕11~14周开展NT超声检查,并于孕22~27周开展四维彩超检查,并以引产或分娩结果为金标准,统计不同检查方法检出胎儿畸形情况及诊断效能。结果:1415例孕妇经金标准检出胎儿畸形48例,NT超声检查与四维彩超各类型畸形检出准确率81.25%、83.33%与引产或分娩结果差异有统计学意义(P<0.05),联合检查各类型畸形检出准确率与引产或分娩结果差异无统计学意义(P>0.05);联合检查诊断胎儿畸形灵敏度、准确率、阴性预测值分别为97.92%、99.93%、99.93%,均高于单一检查(P<0.05)。结论:四维彩超联合NT超声检查在产前胎儿畸形筛查中价值高,便于产前识别发育畸形胎儿,提高新生儿质量。
Objective To explore the application value of four dimensional(4D)color ultrasound combined with nuchal translucency(NT)ultrasound in prenatal screening of fetal malformation.Methods A total of 1415 pregnant women who underwent prenatal screening for fetal malformation in Dongying District People's Hospital of Dongying City from January 2021 to June 2023 were selected as the study objects.NT ultrasound examination was performed at 11~14 weeks of gestation,and 4D color ultrasound examination was performed at 22~27 weeks of gestation,with labor induced or delivery results as the gold standard.The diagnostic efficiency of fetal malformation detected by different examination methods was analyzed.Results Among 1415 pregnant women,48 cases of fetal malformations were detected by gold standard.The accuracy rates of NT ultrasound and 4D color ultrasound were 81.25% and 83.33%,respectively,and the results of labor induction or delivery were significantly different(P<0.05).There was no significant difference between the accuracy rates of combined examination and labor induction or delivery(P>0.05).The sensitivity,accuracy and negative predictive value of combined examination in the diagnosis of fetal malformation were 97.92%,99.93% and 99.93% respectively,which were higher than that of single examination(P<0.05).Conclusion Four dimensional color ultrasound combined with NT ultrasound is of high value in prenatal screening of fetal malformation,which is convenient for prenatal identification of fetal malformation and improvement of newborn quality.
