乳腺癌化疗相关认知障碍患者的脑结构及静息态功能MRI研究进展

Research progress of brain structure and resting-state functional MRI in patients with breast cancer chemotherapy-related cognitive impairment

摘要 乳腺癌患者在接受化疗后常出现化疗相关认知障碍(chemotherapy-related cognitive impairment,CRCI),越来越多的证据表明,CRCI的出现主要是由于大脑结构及功能的改变。MRI可以显示灰质体积/密度的减少、皮质厚度减低、白质微观结构的破坏及脑功能或网络连接的异常。本文将脑灰、白质结构成像及静息态功能MRI(resting state functional magnetic resonance imaging,rs-fMRI)技术在乳腺癌CRCI中的相关研究进展进行综述,主要包括三维T1加权成像(three-dimensional T1 weighted imaging,3D-T1WI)、扩散加权成像(diffusion weighted imaging,DTI)在乳腺癌CRCI患者脑灰、白质异常中的研究和rs-fMRI在乳腺癌CRCI患者的脑低频振幅(amplitude of low frequency fluctuation,ALFF)、局部一致性(regional homogeneity,ReHo)、功能连接(functional connectivity,FC)和脑网络改变方面的研究。评估神经成像标志物与乳腺癌化疗轨迹中的认知功能之间的关联,探索CRCI的神经生物学机制,为CRCI提供相对客观的神经影像学标志物。 Chemotherapy-related cognitive impairment(CRCI)is common among breast cancer patients after chemotherapy.More and more evidence shows that CRCI is mainly due to changes in the brain structure and function.MRI technology can reveal the reduction of gray matter volume/density,the decrease of cortical thickness,the destruction of white matter microstructure,and the abnormalities in brain function or network connectivity.This article aims to review the research on the CRCI of breast cancer using gray matter,white matter structural imaging and resting-state functional MRI(rs-fMRI).It mainly includes three dimensional T1-weighted imaging and diffusion tensor imaging(DTI)on study gray matter and white matter abnormalities,as well as the changes in brain amplitude of low frequency fluctuation(ALFF),regional homogeneity(ReHo),functional connectivity(FC)and brain networks observed in rs-fMRI.To evaluate the association between neuroimaging markers and cognitive function in the trajectory of breast cancer chemotherapy,explore the neurobiological mechanism of CRCI,and provide relatively objective neuroimaging markers for CRCI.
