集成MRI与Kaiser评分对BI-RADS 4类乳腺病变的诊断价值

摘要 目的:探讨集成MRI(SyMRI)定量参数与Kaiser评分(KS)对BI-RADS 4类乳腺病变的鉴别诊断价值。方法:回顾性分析2020年6月至2021年8月在郑州大学第三附属医院术前接受乳腺MRI检查的BI-RADS 4类乳腺病变76例女性患者的临床资料,所有患者均采集SyMRI序列。据患者病理诊断结果分为良性病变32例和恶性病变44例。根据KS评分标准对病变进行评分,测量SyMRI定量参数T1弛豫时间(T1)、T2弛豫时间(T2)和质子密度(PD)值。绘制受试者工作特征(ROC)曲线,比较不同参数对BI-RADS 4类乳腺病变良恶性鉴别的诊断效能。结果:BI-RADS 4类乳腺良性病变的T1、T2值分别为1 865.23(1 519.82,2 232.40) ms、93.24(85.21,128.40) ms,均高于恶性病变[T1、T2值分别是1 295.36(1 139.73,1 519.46) ms、86.48(78.94,90.59) ms](P<0.05)。恶性病变KS高于良性病变(P<0.001)。T1、T2、KS诊断乳腺良恶性病变的ROC曲线下面积(AUC)分别为0.829、0.696、0.855,敏感度分别为81.82%、86.36%、90.91%,特异度分别为75.00%、50.00%、71.87%。T1+KS,T2+KS,T1+T2+KS诊断良恶性病变的敏感度、特异度分别为90.91%、81.25%,90.91%、81.25%,88.64%、75.00%。结论:SyMRI与KS对诊断BI-RADS 4类乳腺病变的良恶性有一定的价值,两者联合不能提高诊断效能。 Aim:To investigate the value of quantitative parameters from synthetic MRI(SyMRI) in combination with Kaiser score(KS) in the differential diagnosis of breast imaging reporting and data system(BI-RADS) 4 category breast lesions.Methods:Clinical data of 76 female BI-RADS 4 category breast lesions(32 benign,44 malignant) patients who underwent breast MRI before surgery from June 2020 to August 2021 in the Third Affiliated Hospital of Zhengzhou University were retrospectively reviewed.All patients underwent SyMRI.KS was assigned and the quantitative parameters T1,T2,and proton density(PD) were measured.The diagnostic efficacy of different parameters in the differential diagnosis of BI-RADS 4 lesions was analyzed based on ROC curve.Results:T1 and T2 values for benign lesions were 1 865.23(1 519.82,2 232.40) ms and 93.24(85.21,128.40) ms,which were significantly higher than those for malignant lesions[1 295.36(1 139.73,1 519.46) ms、86.48(78.94,90.59) ms],respectively(P<0.05).KS of malignant lesions was significantly higher than that of benign lesions(P<0.001).The area under the ROC curve(AUC) for T1,T2,and KS diagnosing of benign and malignant breast lesions was 0.829,0.696,and 0.855,respectively,with sensitivity of 81.82%,86.36%,and 90.91%,and specificity of 75.00%,50.00%,and 71.87%,respectively.The sensitivity and specificity of T1+KS in diagnosing malignant lesions were 90.91%,81.25%,those of T2+KS were 90.91%,81.25% and those of T1+T2+KS were 88.64%,and 75.00%,respectively.Conclusion:SyMRI and KS have certain value in the differential diagnosis of benign and malignant breast lesions which are assigned as BI-RADS category 4,and the combination of the two methods could not significantly improve diagnostic efficiency.
