摘要
手口综合征的主要病因为脑血管疾病,导致“丘脑-皮层环路”和感觉运动网络相关脑区活动的改变。该病病灶在脑,内因是脑部气血受损,五脏气机不调,外因是外感六淫之邪客于肌肤。中风病脑络受损致“神伤”,加之气血虚弱、运行失司,经络受阻引起局部麻木不仁等“形伤”,共同形成其“形神两伤”核心病机,治疗应围绕调和气血、形神共调为主。基于“形神共调”理论,常以督脉上头部穴位百会、神庭、脑户为主穴调神定志,肢体感觉运动功能障碍时,以阳明经上五腧穴为主调形体气血,从而达到调和气血,形神共调之效。针刺手法采用广德张氏芒针疗法,通过循经透刺,一针多穴,直达病所。芒针透刺选穴:头针调神:神庭透百会、百会透脑户;体针调形:合谷透后溪、偏历透曲池、颊车透地仓。
The main Western medical cause of hand-mouth syndrome is cerebrovascular disease,which leads to the change of the activity of the"thalamic-cortical circuit"and the sensory motor network related to the brain.The lesion of the disease is in the brain,internally due to the damage of the brain qi and blood,the imbalance of the five zang organs,and externally due to the six excesses of exogenous sensuality on the skin.Stroke caused by brain damage leads to"spirit injury",combined with"shape injury"due to local numbness resulting from qi and blood deficiency,dysfunction,meridian blockage,etc.,jointly forming the core pathogenesis of"physique and spirit injuries".The treatment should focus on harmonizing qi and blood,and co-regulation of physique and spirit.Based on the theory of"co-regulation of shape and spirit",the head acupoints such as Baihui(DU 20),Shenting(DU 24)and Naohu(DU 17)on the govern vessel(GV)are often used to adjust the spirit of the mind.When the limbs have sensory and motor dysfunction,the five acupoints on the yang brightness meridian are used to adjust the body qi and blood,so as to achieve the effect of harmonizing qi and blood,and co-regulation of physique and spirit.Acupuncture technique uses Family Zhang′s elongated needle therapy in Guangde,through meridian-guided penetration needling,with one needle at multiple acupoints,directly to the disease location.The point selection of penetration needling withe longated needle is as follows:Head needling for adjusting the spirit:Shenting penetrating Baihui,Baihui penetrating Naohu.Body needling for adjusting the physique:Hegu(LI 4)penetrating Houxi(SI 3),Pianli(LI 6)penetrating Quchi(LI 11),Jiache(ST 6)penetrating Dicang(ST 4).
