摘要
目的分析探究血清Lp-PLA2联合CT血管造影(CT angiography,CTA)在糖尿病足下肢动脉病变诊断中的应用。方法选取2020年2月—2022年2月厦门大学附属第一医院收治的100例疑似糖尿病足下肢动脉病变患者为研究对象,以数字减影血管成像技术(Digital Subtraction Angiography,DSA)检查结果为标准,分别对其进行单纯血清Lp-PLA2检测诊断、CTA诊断和血清Lp-PLA2联合CTA诊断,对比三种诊断方式差异,通过ROC曲线评估不同诊断方式对糖尿病足下肢动脉病变的诊断价值。结果血清Lp-PLA2联合CTA联合检测下肢动脉病变的诊断符合率显著高于单纯血清Lp-PLA2、CTA诊断,差异有统计学意义(P<0.05);对于糖尿病足下肢动脉病变的诊断,Lp-PLA2检测与CTA联合的ROC曲线下面积明显高于单项Lp-PLA2检测与单项CTA诊断结果,差异有统计学意义(P<0.05)。结论Lp-PLA2联合CTA检测对糖尿病足下肢动脉病变的诊断有显著的价值。
Objective To investigate the application of serum Lp-PLA2 combined with CT angiography(CTA)in the diagnosis of diabetic foot and lower extremity arterial disease.Methods A total of 100 patients with suspected dia-betic foot and lower extremity arterial disease admitted to the First Affiliated Hospital of Xiamen University from Feb-ruary 2020 to February 2022 were selected as subjects of this study.Based on the results of digital subtraction angiog-raphy(DSA),serum Lp-PLA2 detection,CTA diagnosis and serum Lp-PLA2 combined CTA diagnosis were per-formed respectively.ROC curve was used to evaluate the diagnostic value of different diagnostic methods for diabetic foot and lower extremity arterial disease.Results The diagnostic coincidence rate of serum Lp-PLA2 combined with CTA for lower limb artery lesions was significantly higher than that of serum Lp-PLA2 and CTA alone,and the differ-ence was statistically significant(P<0.05).For the diagnosis of diabetic foot and lower extremity arterial disease,the area under ROC curve of Lp-PLA2 test combined with CTA was significantly higher than that of single Lp-PLA2 test and single CTA diagnosisand,the difference was statistically significant(P<0.05).Conclusion Lp-PLA2 combined with CTA has significant value in the diagnosis of diabetic foot and lower extremity arterial disease.
