摘要
目的:分析GeneXpert MTB/RIF联合痰涂片检查在肺结核诊断中的效能。方法:选取2020年1月至2022年12月于漳州市长泰区接受筛查的152例疑似肺结核患者进行前瞻性研究,均进行痰涂片和GeneXpert MTB/RIF检查,统计单项及联合检查对肺结核的检出结果,并以痰培养结果为金标准,比较单项和联合检查诊断肺结核的效能。结果:痰培养结果显示,152例疑似肺结核患者中,阳性121例,阴性31例;痰涂片检查检出阳性79例,阴性73例;GeneXpert MTB/RIF检查检出阳性112例,阴性40例;联合检查检出阳性122例,阴性30例;GeneXpert MTB/RIF检查诊断肺结核的准确率、灵敏度、特异度、阴性预测值和阳性预测值均高于痰涂片检查,差异有统计学意义(P<0.05);GeneXpert MTB/RIF联合痰涂片检查诊断肺结核的准确率、灵敏度和阴性预测值均高于二者单项检查,差异有统计学意义(P<0.05)。结论:GeneXpert MTB/RIF联合痰涂片检查诊断肺结核的效能优于二者单项检查。
Objective:To analyze efficiency of GeneXpert MTB/RIF combined with sputum smear in diagnosis of pulmonary tuberculosis.Methods:A prospective study was conducted on 152 patients with suspected pulmonary tuberculosis who were screened in Changtai district of Zhangzhou city from January 2020 to December 2022.All patients underwent sputum smear and GeneXpert MTB/RIF examination.The detection results of single and combined examinations for pulmonary tuberculosis were statistically analyzed.Using the results of sputum culture as the gold standard,the efficiencies of single and combined examinations in the diagnosis of pulmonary tuberculosis were compared.Results:The sputum culture showed that among the 152 patients with suspected pulmonary tuberculosis,121 were positive and 31 were negative.The sputum smear examination showed positive in 79 cases and negative in 73 cases.GeneXpert MTB/RIF examination detected 112 positive cases and 40 negative cases.Combined examination detected 122 positive cases and 30 negative cases.The accuracy,the sensitivity,the specificity,the negative predictive value and the positive predictive value of GeneXpert MTB/RIF in the diagnosis of pulmonary tuberculosis were higher than those of sputum smear examination,and the differences were statistically significant(P<0.05).The accuracy,the sensitivity and the negative predictive value of GeneXpert MTB/RIF combined with sputum smear examination in the diagnosis of pulmonary tuberculosis were higher than those of the two single examinations,and the differences were statistically significant(P<0.05).Conclusions:The efficiency of GeneXpert MTB/RIF combined with sputum smear examination in the diagnosis of pulmonary tuberculosis is superior to those of the two single examinations.
