摘要
目的编制适用于成人体外循环手术压力性损伤的风险评估工具,便于护理人员及时、准确、动态地进行评估,并适时采取针对性的预防措施,以降低体外循环手术压力性损伤的发生率。方法参考已开发的压疮评估量表及国内部分改良和自制量表,结合成人体外循环手术的特点,编制成人体外循环手术压力性损伤评估量表,采用随机抽样法选择110例手术患者进行压疮风险评估,用信效度评估工具对量表的信度和效度进行评价。结果信度检验:该量表的内在一致性信度克朗巴赫系数为0.787,3个维度分别为0.761,0.728,0.779,分半信度为0.703。效度检验:(1)内容效度:该量表条目内容效度指数(I-CVI)为0.81~1,总体内容效度指数(S-CVI)为0.94;(2)结构效度:通过探索性因素分析共萃取3个公因子,累积方差贡献率为63.278%,每个条目的因素负荷均大于0.4。结论(1)初步完成了成人体外循环手术压力性损伤风险评估工具的编制;(2)编制的评估工具具有良好的信效度,临床实用性强,符合国人人群特点。
Objective To develop a stress injury risk assessment tool applicable to adult extracorporeal circulation surgery patients,which enable nursing staff to make timely,accurate,and dynamic assessments and take targeted preventive measures appropriate to reduce the incidence of stress injury in extracorporeal circulation surgery.Methods With reference to the developed pressure ulcer assessment scale and some modified and homemade scales in China,the adult pressure injury assessment scale for extracorporeal circulation surgery was developed by combining the characteristics of adult extracorporeal circulation surgery.The scale was used in a survey involved 110 patients for pressure ulcer risk assessment with random sampling method,and the reliability and validity of the scale were evaluated with the reliability assessment tool.Results Reliability test:The internal consistency reliability Cronbach coefficient of the Scale was 0.787,the three dimensions were 0.761,0.728,and 0.779,and the split-half reliability was 0.703.Validity test:(1)Content validity:The content validity index(I-CVI)of the Scale entries ranged from 0.81 to 1,and the overall content validity index(S-CVI)was 0.94;(2)Construct validity:A total of three common factors were extracted by exploratory factor analysis,and the cumulative variance contribution rate was 63.278%,and the factor loading of each entry was greater than 0.4.Conclusions(1)The development of a risk assessment tool for pressure injury in adults undergoing extracorporeal circulation surgery was initially completed;(2)The developed assessment tool has good reliability and validity,which is clinically practical.It is suitable for people with typical Chinese characteristics.
