摘要
目的分析可溶性髓样细胞触发受体-1(sTREM-1)、Ⅱ型胶原C端肽(CTX-Ⅱ)及血沉(ESR)对膝关节炎化脓性感染的诊断价值。方法以2019年7月-2022年8月江苏省淮安市洪泽区人民医院收治的106例膝关节炎患者作为膝关节炎组,选取同期在医院进行体检的112名健康体检者作为健康对照组,统计两组临床资料、血清sTREM-1、CTX-Ⅱ水平及ESR;根据膝关节炎患者是否发生化脓性感染将其分为化脓性感染组(44例)和非化脓性感染组(62例),比较两组血清sTREM-1、CTX-Ⅱ水平及ESR,采用受试者工作特征(ROC)曲线分析血清sTREM-1、CTX-Ⅱ及ESR单独及联合对膝关节炎患者化脓性感染的预测价值。结果膝关节炎组血清sTREM-1、CTX-Ⅱ水平及ESR高于健康对照组(P<0.05);化脓性感染组血清sTREM-1、CTX-Ⅱ水平及ESR高于非化脓性感染组(P<0.05);血清sTREM-1、CTX-Ⅱ及ESR联合检测预测膝关节炎患者化脓性感染曲线下面积(AUC)值高于三者单独检测(P<0.05);血清sTREM-1、ESR及联合预测膝关节炎患者化脓性感染的敏感度高于CTX-Ⅱ,ESR高于联合(P<0.05);血清sTREM-1、CTX-Ⅱ及联合预测膝关节炎患者化脓性感染的特异度高于ESR,CTX-Ⅱ及联合高于sTREM-1(P<0.05)。结论膝关节炎患者化脓性感染的发生可引起血清sTREM-1、CTX-Ⅱ水平及ESR水平变化,同时三者联合对膝关节炎患者化脓性感染具有较好的预测价值,其有望成为预测膝关节炎患者化脓性感染的标志物。
OBJECTIVE To analyze the diagnostic value of soluble myeloid cell trigger receptor-1(sTREM-1),type Ⅱ collagen C-terminal peptide(CTX-Ⅱ)and erythrocyte sedimentation rate(ESR)in suppurative infection of knee arthritis.METHODS A total of 106 patients with knee arthritis admitted to Hongze District People′s Hospital of Huai an City in Jiangsu Province from Jul 2019 to Aug 2022 were enrolled as the knee arthritis group,and 112 healthy subjects under physical examination during the same period were in the healthy control group.Clinical data,serum sTREM-1,CTX-Ⅱ and ESR levels between the two groups were analyzed.Patients with knee arthritis were divided into the suppurative infection group(44 cases)and non-suppurative infection group(62 cases)according to whether suppurative infection occurred.Serum levels of sTREM-1,CTX-Ⅱ and ESR were compared between the two groups.Receiver operating characteristic curve(ROC)curve was used to analyze the individual and combined predictive value of serum sTREM-1,CTX-Ⅱ and ESR in suppurative infection in patients with knee arthritis.RESULTS The levels of serum sTREM-1,CTX-Ⅱ and ESR in the knee arthritis group were higher than those in the healthy control group(P<0.05).The levels of sTREM-1,CTX-Ⅱ and ESR in the suppurative infection group were higher than those in the non-suppurative infection group(P<0.05).The area under the curve(AUC)of the combined detection of serum sTREM-1,CTX-Ⅱ and ESR in the diagnosis of suppurative infection in patients with knee arthritis was higher than that of the three tests individual(P<0.05).The sensitivity of serum sTREM-1,ESR and combination to predict suppurative infection in patients with knee arthritis was higher than that of CTX-Ⅱ;and that of ESR was higher than that of the combined detection(P<0.05).The specificity of serum sTREM-1,CTX-Ⅱ and combined in the diagnosis of suppurative infection in knee arthritis patients was higher than that of ESR;and that of the combination of CTX-Ⅱ and sTREM-1 was higher than that of STREM-1(P<0.05).CONCLUSION The occurrence of suppurative infection in patients with knee arthritis can cause variation levels of sTREM-1,CTX-Ⅱ and ESR,and the combined detection of the three indexes has good predictive value for suppurative infection in patients with knee arthritis,and it is expected to become a marker for predicting suppurative infection in patients with knee arthritis.
