盐酸羟考酮对舌癌根治术患者术后血流动力学和氧化应激反应的影响

Effect of Oxycodone Hydrochloride on Postoperative Hemodynamics and Oxidative Stress Response in Patients Undergoing Radical Tongue Cancer Surgery

摘要 目的 探讨盐酸羟考酮对舌癌根治术患者术后血流动力学和氧化应激反应的影响。方法 选取医院2021年2月至2022年2月行舌癌根治术的舌癌患者113例,按随机数字表法分为观察组(57例)和对照组(56例)。两组患者均采用舒苏太尼复合咪达唑仑进行麻醉诱导,右美托咪定复合瑞芬太尼进行麻醉维持。术后,观察组给予盐酸羟考酮自控镇痛泵,对照组给予舒芬太尼自控镇痛泵。结果 术后2,6,24 h,观察组患者的平均动脉压、心率均显著低于对照组(P <0.05);观察组患者的苏醒期血清血红素加氧酶-1、超氧化物歧化酶水平均显著高于对照组(P <0.05),丙二醛水平显著低于对照组(P <0.05);观察组患者术毕警觉-镇静分级量表(OAA/S)评分显著高于对照组(P <0.05),疼痛数字等级评定量表(NRS)评分显著低于对照组(P <0.05);两组患者的苏醒时间、不良反应发生率相当(P> 0.05)。结论 盐酸羟考酮用于舌癌根治术患者的术后镇痛,可有效缓解患者的术后疼痛程度,减轻氧化应激反应,稳定围术期血流动力学水平,且不影响麻醉苏醒时间,安全性高。 Objective To investigate the effect of oxycodone hydrochloride on postoperative hemodynamics and oxidative stress response in patients undergoing radical tongue cancer surgery.Methods A total of 113 patients with tongue cancer who underwent radical surgery in the hospital from February 2021 to February 2022 were selected and randomly divided into the observation group(57 cases)and the control group(56 cases)by the random number table method.The patients in the two groups were anesthetized with sufentanil combined with midazolam for induction,and they were given dexmedetomidine combined with remifentanil for anesthesia maintenance.After surgery,the patients in the observation group were given the patient-controlled analgesia(PCA)with oxycodone hydrochloride,while the patients in the control group were given the PCA with sufentanil.Results At 2,6,and 24 h after surgery,the average arterial pressure and heart rate in the observation group were significantly lower than those in the control group(P<0.05).During the recovery period,the serum levels of heme oxygenase(HO)-1 and superoxide dismutase(SOD)in the observation group were significantly higher than those in the control group(P<0.05),while the level of malondialdehyde(MDA)in the observation group was significantly lower than that in the control group(P<0.05).At the end of the surgery,the Postoperative Observers Assessment of Alertness/Sedation Scale(OAA/S)score in the observation group was significantly higher than that in the control group,while the Numerical Pain Rating Scale(NRS)score in the observation group was significantly lower than that in the control group(P<0.05).The recovery time and incidence of adverse reactions were comparable between the two groups(P>0.05).Conclusion Oxycodone hydrochloride can effectively relieve the postoperative pain for patients undergoing tongue cancer radical surgery,alleviate oxidative stress reactions,stabilize perioperative hemodynamic levels,and does not affect anesthesia recovery time,with high safety.
