摘要
目的行头颈部CT血管造影术(CT angiography,CTA)检查患者在推注对比剂时采用不同流率干预方案,分析其在改善图像质量中所起到的作用。方法选取2023年1—6月南京市第一医院接受头颈部CTA检查的242例患者,以随机数表法分为A组(121例)与B组(121例),所用对比剂均为碘帕醇(370 mg(I)/mL),用量60 mL,且均在右肘前静脉处进行注入操作,注射流率均控制在5 mL/s,追加70 mL的生理盐水,A组生理盐水注入流率为5 mL/s,B组为4 mL/s。对比两组图像质量、辐射剂量。结果A组血管与周围结构对比度[(4.34±0.20)分]、管腔边缘锐利度[(4.19±0.17)分]以及图像清晰度[(4.11±0.12)分]均高于B组[(4.25±0.14)分、(4.12±0.15)分与(4.05±0.10)分],差异有统计学意义(P<0.05)。两组噪声指标对比,差异无统计学意义(P>0.05)。两组有效辐射剂量、CT剂量长度乘积、CT剂量指数对比,差异无统计学意义(P>0.05)。结论在对比剂推注时,通过加快追加生理盐水的流率,可获得更好的图像质量,在实际操作时,以盐水的冲洗流率与对比剂的流率相同最合适。
Objective To analyze the role played by different flow-rate intervention protocols in improving image quality in patients undergoing head and neck CT angiography(CTA)during contrast injection.Methods A total of 242 patients who underwent head and neck CTA examination in the First Hospital of Nanjing City from January to June 2023 were selected and divided into group A(121 cases)and group B(121 cases)by the random number table method,and the contrast agent used was iopamidol(370 mg(I)/mL)with a dosage of 60 mL,and they were all injected into the anterior vein of the right elbow,and the flow rate of the injections was controlled to be 5 mL/s,and an additional 70 mL of saline was added,and the injection flow rate of saline in group A was 5 mL/s,and that in group B was 4 mL/s.The image quality and radiation dose of the two groups were compared.Results The contrast between vessels and surrounding structures[(4.34±0.20)points],lumen edge sharpness[(4.19±0.17)points],and image clarity[(4.11±0.12)points]in group A were higher than those in groups B[(4.25±0.14)points,(4.12±0.15)points,and(4.05±0.10)points],and the difference was statistically significant(P<0.05).The noise indexes of the two groups,the difference was not statistically significant(P>0.05).Compared the effective radiation dose,CT dose length product,and CT dose index between the two groups were compared,the difference was not statistically significant(P>0.05).Conclusion During the bolus injection of contrast agent,better image quality can be obtained by accelerating the flow rate of additional physiological saline.In actual operation,it is most appropriate to have the flushing flow rate of saline and the flow rate of contrast agent be the same.
