摘要
目的 探讨对肾铸型结石患者采用智能控压负压吸引输尿管软镜钬激光碎石术治疗的临床效果。方法 选取2020年1月—2022年12月靖江市中医院收治的70例肾铸型结石患者为研究对象,根据随机数表法分为参照组和研究组,各35例。参照组以常规输尿管软镜碎石术治疗;研究组以智能控压负压吸引输尿管软镜钬激光碎石术治疗;对比两组患者的结石清除率、住院总费用、手术时间、术后血白细胞数量、住院时间、并发症(腰痛、发热、肾包膜下血肿)总发生率。结果 研究组结石清除率明显高于参照组,差异有统计学意义(P<0.05);研究组住院总费用为(24 215.88±3 525.25)元,低于参照组的(26 497.85±4230.13)元,差异有统计学意义(P<0.05);研究组手术时间(78.25±20.28)min与参照组的(60.85±12.25)min相比明显更长,差异有统计学意义(P<0.05);参照组术后血白细胞数量和住院时间为(9.85±1.25)×10^(9)个/L和(8.23±1.95)d,研究组为(9.77±1.17)×10^(9)个/L和(8.19±1.98)d,两组比较,差异无统计学意义(P>0.05);两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论 智能控压负压吸引输尿管软镜钬激光碎石术治疗肾铸型结石患者,虽然导致手术时间有所延长,但能提高结石清除率,减少患者的住院费用,并且不会导致并发症增加,可促进肾铸型结石患者的良好预后。
Objective To investigate the clinical effect of holmium laser lithotripsy with intelligent controlled negative pressure suction ureteral soft scope for patients with renal cast stones.Methods A total of 70 patients with renal cast stones received in Jingjiang Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were selected as the study objects.All patients with renal cast stones were divided into control group and study group based on random number table method,35 cases in each group.The control group was treated with conventional ureteral soft scope lithotripsy,and the study group was treated with intelligent controlled negative pressure suction ureteral soft scope holmium laser lithotripsy.The calculus clearance rate,total hospitalization cost,operation time,postoperative white blood cell count,hospital stay,and total incidence of complications(lumbago,fever,subcapsular hematoma)were compared between the two groups.Results Compared with the control group,the calculative results of renal cast stone patients in the study group were significantly higher,the difference was statistically significant(P<0.05).Compared with the control group(26497.85±4230.13)yuan,the total inpatient cost of the study group(24215.88±3525.25)yuan was significantly lower,the difference was statistically significant(P<0.05).Compared with the control group(60.85±12.25)min,the operation time of the study group(78.25±20.28)min was significantly longer,the difference was statistically significant(P<0.05).There was no significant difference in the number of white blood cells and the length of hospital stay between the two groups[(9.85±1.25)×10^(9)/L vs(9.77±1.17)×10^(9)/L]and[(8.23±1.95)d vs(8.19±1.98)d](P>0.05).There was no statistically significantly different between the two groups in the total incidence of complications(P>0.05).Conclusion Compared with conventional ureteral soft scope lithotripsy,the effective application of intelligent controlled pressure negative pressure aspiration ureteral soft scope lithotripsy during the treatment of renal cast stones can significantly increase the stone clearance rate,significantly reduce the patient's hospitalization cost,and do not lead to increased complications,although the operation time is extended.It can promote the good prognosis of patients with cast kidney stones.
