摘要
目的评价参松养心胶囊联合盐酸胺碘酮片治疗冠心病室性早搏的临床疗效及对一氧化氮(Nitric oxide,NO)和血清血栓素(Serum thromboxin,TXB2)含量的影响研究。方法选取医院收治的126例冠心病室性早搏患者,随机分为观察组和参照组,每组63例。两组均给予冠心病基础治疗,参照组口服盐酸胺碘酮片,第1周,1次0.2 g,3次/d;第2周,1次0.2 g,2次/d;第3周,1次0.2 g,1次/d。研究组患者采用参松养心胶囊联合盐酸胺碘酮片治疗,盐酸胺碘酮片服用方式同参照组,参松养心胶囊1次3粒,3次/d。两组持续治疗3周,比较两组治疗后疗效、证候积分、心功能指标、血清NO、TXB2水平变化。结果观察组疗效优于参照组(P<0.05);两组治疗前畏寒肢冷和面色无华等中医证候积分无差异(P>0.05);两组治疗后证候积分明显较低(P<0.05),且观察组降低较明显(P<0.05);两组心功能指标明显改善(P<0.05),且观察组改善幅度优于参照组(P<0.05);两组NO、TXB2浓度水平改善明显(P<0.05),改善幅度方面观察组较参照组更优(P<0.05)。结论参松养心胶囊联合盐酸胺碘酮片治疗冠心病室性早搏疗效确切,可改善冠心病室性早搏患者气虚血瘀证的症状,减轻心肌损伤,增强左心室功能,提高治疗疗效,值得临床进一步研究探讨。
Objective To evaluate the clinical efficacy of Shensong Yangxin Capsule(参松养心胶囊)combined with amioda⁃rone hydrochloride tablet in the treatment of premature ventricular beats of coronary heart disease and the effects of nitric oxide(NO)and thromboxin(TXB2)in serum.Methods A total of 126 patients with premature ventricular beats of coronary heart dis⁃ease admitted to the hospital were randomly divided into observation group and control group,with 63 cases in each group.Both groups were given basic treatment for coronary heart disease.The control group was given amiodarone hydrochloride tablets,0.2 g/time,3 times a day in the first week;0.2 g/time,twice a day in the second week;0.2 g/time,once a day in the third week.The patients in the study group added Shensong Yangxin Capsule(3 capsules/time,3 times/d).The amiodarone hydrochloride tablets were administered in the same way as that in the control group.Both groups were treated for 3 weeks.The total effective rate was compared,and the levels of cardiac function indexes[left ventricular ejection fraction(LVEF),left ventricular end-sys⁃tolic dimension(LVESD),left ventricular end-diastolic dimension(LVEDD)],NO and TXB2 were compared.Results The ther⁃apeutic effect in the observation group was better than that of the control group(P<0.05).Before treatment,there was no differ⁃ence in the scores of slight cold limbs or complexion between the two groups(P>0.05).After treatment,the syndrome scores of the two groups were significantly lower(P<0.05),and the scores of the observation group were significantly lower(P<0.05).The heart function indexes of the two groups were significantly improved(P<0.05),and the improvement of the study group was better(P<0.05).The levels of NO and TXB2 in the two groups were significantly improved(P<0.05)and the improvement in the study group was better(P<0.05).Conclusion The combined medication is effective in treating premature ventricular beats of coronary heart disease.It can improve the symptoms of Qi deficiency and blood stasis syndrome in patients with premature ventricular beats of coronary heart disease,reduce the myocardial injury,improve the autonomic nervous function imbalance,enhance the left ventricular function and improve the therapeutic effect.
