葛根素对背根神经病理性疼痛大鼠的镇痛作用机制研究

摘要 目的探究葛根素对背根神经病理性疼痛大鼠的镇痛作用机制。方法将120只大鼠随机平均分为三组,A组大鼠采用背根神经结扎手术,B组大鼠采用鞘内注射葛根素联合背根神经结扎手术,C组大鼠不结扎背根神经,其他手术过程同A组。检测大鼠术后1、3、5和7 d的机械缩足反应阈值和热缩足潜伏期;显微镜下观察背根神经元细胞数量与形态,用western blotting和酶联免疫吸附法检测大鼠背根神经炎性因子白细胞介素-6(IL-6)和白细胞介素-1β(IL-1β)表达。结果术后1、3、5和7d,A组、B组大鼠机械缩足反应阈值与C组相比持续下降(P<0.05);术后3、5和7 d,B组大鼠机械缩足反应阈值均高于A组(P<0.05);术后5 d和7 d,A组、B组大鼠热缩足潜伏期均低于C组(P<0.05);术后7d,B组大鼠的背根神经元细胞数量较多,背根神经节IL-6、IL-1β蛋白表达、染色情况评分高于A组、C组(P<0.05)。结论鞘内注射葛根素可有效减轻神经损伤引起的机械热痛觉过敏和痛觉超敏,并可能通过降低背根神经内的IL-6和IL-1β表达改善神经病理性疼痛。 Objective To explore the analgesic effect of puerarin on dorsal root neuropathic pain in rats.Methods A total of 120 rats were randomly assigned to three groups evenly,group A received spinal nerve ligation surgery(SNLS),B did intrathecal injection of puerarin combined with the SNLS,C did the same operation contents as A but didn't ligate spinal nerve after nerve exposure.Mechanical withdrawal thresholds(MWT)and thermal withdrawal latencies(TWL)were detected on day 1,3,5 and 7 after operation;the number and morphology of dorsal root neurons were observed under microscope,interleukin-6(IL-6)and IL-16 levels detected using western blotting and enzyme-linked immunosorbent assay(ELISA),and IL-6 and IL-1βexpression levels in dorsal root ganglia measured using the ELISA.Results On day 1,3,5 and 7 after operation,the MWTs continuously declined in group A and B compared with C(P<0.05);on day 3,5 and 7 after operation,MWTs were higher in group B than A(P<0.05);on day 5 and 7 after operation,the TWLs were lower in group A and B than C(P<0.05);on day 7 after operation the numbers of dorsal root neuron cells were more as well as IL-6 and IL-1βexpression levels in dorsal root ganglia and scores on dyeing condition higher in group B than C and A(P<0.05).Conclusion Intrathecal injection of puerarin could effectively alleviate mechanical thermal hyperalgesia and pain hypersensitivity caused by nerve injury and may improve neuropathic pain via decreasing IL-6 and IL-1βlevels in dorsal root nerve.
