摘要
目的探讨血常规中红细胞相关指标及网织红细胞绝对值联合铁指标在诊断缺铁性贫血(Iron Deficiency Anemia,IDA)的价值。方法回顾性收集我院84例诊断为缺铁性贫血的患者作为缺铁贫组,61例经PCR诊断为地中海贫血的患者为地贫组以及100例体检健康人群作为健康对照组,检测各组的红细胞计数(RBC)、血红蛋白浓度(HGB)、红细胞平均体积(MCV)、红细胞平均血红蛋白量(MCH)、红细胞分布宽度(RDW)、网织红细胞绝对值(RET#)、铁蛋白(FER)、血清铁(FE)、转铁蛋白(TF)进行统计学比较。对各组有差异的指标进行ROC曲线分析评价其诊断效能,得出其灵敏度、特异度。结果缺铁贫组和地贫组的HGB、MCV和MCH皆低于健康对照组(P<0.05);缺铁贫组的RDW相对地贫组和健康对照组均高(P<0.05);缺铁贫组的RBC、FER、FE相对地贫组和健康对照组均低(P<0.05);地贫组的RBC和RET#相对缺铁贫组和健康对照组高(P<0.05)。三组数据TF差异无统计学意义。RBC(AUC:0.749)、RDW(AUC:0.901)、FER(AUC:0.951)、FE(AUC:0.984)指标对诊断IDA具有较好的灵敏度和特异性。结论RBC、RDW联合FER、FE指标对IDA的诊断意义重大,RET#不作为诊断缺铁性贫血IDA的指标。
Objective To investigate the value of the routine blood detection by joint absolute number of reticulocytes and iron index in iron deficiency anemia.Methods Eighty-four patients diagnosed with iron deficiency anemia in our hospital were retrospectively collected as Group Iron Deficiency Anemia,61 patients diagnosed with thalassemia by PCR as Group Thalassemia,and 100 healthy people who underwent physical examination as the Healthy control group.Detect each group of red blood cell count(RBC),hemoglobin concentration(HGB),average red blood cell volume(MCV),red blood hemoglobin(MCH),red blood cell distribution width(RDW)and reticulocyte absolute value(RET#),ferritin(FER),serum iron(FE),transferrin(TF)between groups compared statistically.The ROC curve was used to evaluate the diagnostic efficacy of the different indicators in each group,and obtained the sensitivity and specificity.Conclusion HGB,MCV and MCH of iron deficiency anemia group and thalassemia group were lower than that of healthy control group(P<0.05).The RDW of the iron deficient group was higher than that of the iron deficient group and the healthy control group(P<0.05).RBC,FER and FE in the poor group were lower than those in the poor group and the healthy control group(P<0.05).RBC's and RET#were higher in the destitute group than in the iron deficient group and the healthy control group(P<0.05).There was no statistical significance in pairwise comparison of three groups of data TF.RBC(AUC:0.749),RDW(AUC:0.901),FER(AUC:0.951),FE(AUC:0.984)had better sensitivity and specificity for the diagnosis of IDA.Results RBC、RDW combined with FER、FE has reference significance for the diagnosis of Iron Deficiency Anemia,while RET#has no reference significance for the diagnosis of iron deficiency anemia.
