摘要
目的基于中医传承辅助平台(V2.5)探讨谢晶日教授治疗慢性萎缩性胃炎(chronic atrophic gastritis,CAG)伴肠化生(intestinal metaplasia,IM)的用药经验。方法收集筛选出2020年1月—2022年1月就诊于谢晶日教授门诊且确诊为CAG伴IM的病例,提取并规范首诊处方,录入中医传承辅助平台,进行频次统计分析、组方规律分析以及新方分析,根据分析结果探讨谢师治疗本病的用药经验。结果该研究共纳入76首处方,包含104味中药,药性以温性、寒性为主,药味以苦、辛居多,归经多属于脾、胃、肺、肝经,功效频次以理气药和补虚药居前。通过组方规律分析,获得11味核心药物,包括白术、柴胡、神曲等;提取65个核心药物组合,其中“白术、柴胡”频次最高;提取103条核心药物关联规则,其中“白花蛇舌草,半枝莲”关联性最强。通过新方分析,获得5首治疗CAG伴IM的新处方。结论谢晶日教授治疗CAG伴IM以疏肝健脾和胃为主,兼以清热解毒化瘀;用药以白术、柴胡、神曲、陈皮、半枝莲、紫苏子、白花蛇舌草、白豆蔻、香橼、海螵蛸、乌药为核心药物,随症加减,诸药同用,标本兼治,体现出谢师在该病治疗过程中灵活辨治的思想。
Objective To explore Professor XIE Jingri's medication experience in treating chronic atrophic gastritis(CAG)with intestinal metaplasia(IM)based on Traditional Chinese Medicine Inheritance Support Platform(V2.5).Methods It collected and screened cases of CAG with IM treated by professor XIE Jingri from January 2020 to January 2022.The first visit prescriptions were extracted and standardized and then input into Traditional Chinese Medicine Inheritance Support Platform.It conducted fre-quency statistical analysis,group rule analysis and new prescription analysis,and discussed the medication experience of Professor XIE Jingri according to the analysis results.Results In this study,76 prescriptions were included,including 104 traditional Chi-nese drugs.The drugs were mainly warm and cold,and the drugs were mostly bitter and spicy.Most of them belonged to the spleen,stomach,lung and liver meridians,and the frequency of drug efficacy was dominated by regulating Qi drugs and tonifying deficiency drugs.Through formation rule analysis,11 core drugs,including Baizhu(Atractylodis Macrocephalae Rhizoma),Chaihu(Bupleuri Radix)and Shenquu(medicated leaven)were obtained.65 core drug combinations were extracted,among which Baizhu(Atractylodis Macrocephalae Rhizoma)and Chaihu(Bupleuri Radix)had the highest frequency.103 core drug association rules were extracted,among which Baihuasheshecao(Hedyotidis Herba)and Banzhilian(Scutellariae Barbatae Herba)were the most strongly related.Five new prescriptions for treating CAG with IM were obtained through the new square analysis.Conclusion Pro-fessor XIE Jingri treated CAG with IM mainly by relieving liver,spleen and stomach,and also by clearing heat,detoxification and blood stasis.With Baizhu(Atractylodis Macrocephalae Rhizoma),Chaihu(Bupleuri Radix),Shenquu(medicated leaven),Chenpi(Citri Reticulatae Pericarpium)、Banzhilian(Scutellariae Barbatae Herba),Zicuzi(Perillae Fructus),Baihuasheshecao(Hedyoti-dis Herba),Baidoukou(Amomi Fructus Rotundus)、Xiangyuan(Citri Fructus)and Haipiaoxiao(Sepiae Endoconcha)as the core drugs.The medication should be adjusted according to the syndrome differentiation to treat both the symptoms and root causes,re-flecting the idea of Professor XIE in the treatment of this disease.
