摘要
目的探讨评估-沟通-监测-患者-环境(ACMPE)管理模式在老年2型糖尿病患者社区护理中的应用价值。方法选取2019年1月至2021年12月高安市骨伤医院管辖的社区内诊治的100例老年2型糖尿病患者作为研究对象,按随机数字表法分为对照组和试验组,各50例。对照组采用常规社区管理模式,试验组采用ACMPE管理模式,干预6个月后对两组患者的血糖控制情况、自我管理能力以及生命质量进行比较,同时统计两组患者并发症发生情况。结果与对照组比较,试验组干预6个月后空腹血糖、餐后2 h血糖、糖化血红蛋白水平均明显降低(P<0.05);与对照组比较,试验组干预6个月后自我管理能力各维度评分均明显升高(P<0.05);试验组干预6个月后生命质量各维度评分低于对照组(P<0.05);试验组并发症发生率低于对照组(P<0.05)。结论与常规管理措施比较,ACMPE管理模式更利于改善老年2型糖尿病患者的血糖控制情况,提高自我管理能力及生命质量,降低并发症发生风险,具有较高的临床应用价值。
Objective To explore the application value of evaluation-communication-Monitoring-patient-environment(ACMPE)management model in community nursing of elderly patients with type 2 diabetes.Methods A total of 100 elderly patients with type 2 diabetes admitted to Gaoan Bone Injury Hospital from January 2019 to December 2021 were selected as the study objects,and were divided into control group and experimental group according to random number table method,with 50 cases in each group.The control group adopted the conventional community management mode,and the experimental group adopted the ACMPE management mode.After 6 months of intervention,the blood glucose control,self-management ability and quality of life of the two groups of patients were compared,and the incidence of complications of the two groups were counted.Results Compared with the control group,the fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin levels of experimental groups were significantly decreased after 6 months of intervention(P<0.05).Compared with the control group,the scores of all dimensions of self-management ability of experimental groups were significantly increased after 6 months of intervention(P<0.05).After 6 months of intervention,the scores of all dimensions of quality of life in experimental groups were lower than those in control group(P<0.05).The complication rate of experimental group was lower than that of control group(P<0.05).Conclusion Compared with conventional management measures,ACMPE management mode is more conducive to improving blood glucose control in elderly patients with type 2 diabetes,improving self-management ability and quality of life,and reducing the risk of complications,which has high clinical application value.
