EV-A71疫苗接种后手足口病流行及临床特点

摘要 目的探讨肠道病毒71型(EV-A71)疫苗接种后手足口病的流行及临床特点。方法收集2018年1月至2021年12月杭州市儿童医院688例肠道病毒感染手足口病患儿的临床资料,分析EV-A71疫苗接种史的手足口病的病原学及临床特点。结果共有EV-A71疫苗接种史手足口病患儿47例(接种一次疫苗11例,接种二次疫苗36例)。其中男28例,女19例;年龄8个月~4岁。<1岁5例,1~3岁40例,>3~4岁1例。临床表现:47例均发热,口、咽、手、足和臀部出疹分别47例、36例、33例、32例;颜面、颈部、四肢、躯干、肘部等其他部位出皮疹25例。47例疫苗后手足口病中,肠道病毒A6型(CV-A6)感染31例,肠道病毒A16型(CV-A16)感染9例,其他型肠道病毒感染7例,无EV-A71感染。血清EV-A71 IgM抗体阳性14例,其中阳性3例,弱阳性11例,接种1次EV-A71疫苗6例,接种2次EV-A71疫苗8例,接种与发病时间最长12个月。结论全面接种EV-A71疫苗能有效预防EV-A71感染,建议积极推广。 Objective To investigate the epidemic and clinical characteristics of hand-foot-and-mouth disease(HFMD)after Enterovirus A71(EV-A71)vaccination.Methods 688 children with enterovirus infection HFMD diagnosed in the laboratory and hospitalized in Hangzhou children's Hospital from January 2018 to December 2021 were collected.Investigated and summarized the etiology and clinical characteristics of HFMD with EV-A71 vaccination history.Results There were 47 cases of HFMD with EV-A71 vaccination history(11 cases were vaccinated once and 36 cases were vaccinated twice).There were 28 males and 19 females.Age distribution:5 cases were less than 1 year old,40 cases were 1-3 year old,1 case was 3-4 year old.Clinical manifestations:47 cases were all fever with oral herpes,36 cases of herpes in the isthmus,33 cases of herpes in the hands,32 cases of herpes in the feet and buttocks.There were 25 cases of rashes on the face,neck,limbs,trunk,elbow and other parts.Among 47 cases of HFMD after vaccination,31 cases were infected by enterovirus A6(CV-A6),9 cases by enterovirus A16(CV-A16),7 cases by other enteroviruses,and 0 cases by EV-A71.The serum EV-A71 IgM antibody was positive in 14 cases,including 3 cases positive and 11 cases weakly positive.Among the 14 patients,6 received EV-A71 vaccine once and 8 received EV-A71 vaccine twice.The longest time from vaccination to infection was 12 months.Conclusion EV-A71 vaccination can effectively prevent EV-A71 infection,but it can not prevent other non EV-A71 enteroviruses.
